摘要
目的研究高血压前期患者动脉弹性及颈总动脉内膜中层厚度(IMT)与机体糖脂代谢异常的关系。方法选取2016年1月至2017年12月60例高血压前期患者为高血压前期组,同期1-2级高血压患者60例为高血压组,血压正常志愿者60例为对照组,比较三组研究对象动脉弹性指数C1、C2、IMT及各组血脂、血糖、胰岛素抵抗指数,分析高血压前期病变与糖脂代谢异常的关系。结果对照组、高血压前期组、高血压组C1、C2依次降低,IMT、FPG、IR依次升高,高血压前期组、高血压组两组的TG、TC无统计学差异(P>0.05),且均高于对照组,差异均有统计学意义(P<0.05);相关性分析:C1与FPG(r=-0.521,P=0.042)、IR(r=-0.562,P=0.035)呈负相关,C2与FPG(r=-0.672,P=0.031)、IR(r=-0.596,P=0.021)呈负相关,IMT与FPG(r=0.614,P=0.023)、IR(r=0.672,P=0.004)呈正相关,均有统计学意义(P<0.05);C1、C2、IMT与TG、TC无相关性(P>0.05)。结论糖脂代谢异常与高血压前期病变互为因果,高血压前期病变的防治、以及对糖脂代谢异常的纠正在高血压的预防上有重要的临床意义。
Objective to study the relationship between arterial elasticity,carotid artery intima media thickness(IMT)and abnormal glucose and lipid metabolism in prehypertensive patients.Methods from January 2016 to December 2017,60 prehypertensive patients were set up as prehypertension group,60 patients with 1-2grade hypertension were set up as hypertension group and 60 healthy volunteers were used as control group.The arterial elasticity index C1,C2,IMT,blood lipid,blood glucose and insulin resistance index were compared in the three groups.The relationship between prehypertension and abnormal glucose and lipid metabolism.Results the C1 and C2in the control group,prehypertensive group and hypertension group decreased in turn,and IMT,FPG and IR increased in turn.There was no statistical difference between the two groups of pre hypertension group and hypertension group,TG and TC(P>0.05),which were all higher than those in the control group.The difference was statistically significant(P<0.05),Correlation analysis:C1was negatively correlated with FPG(r=-0.521,P=0.042),IR(r=-0.562,P=0.035),C2 was negatively correlated with FPG(r=-0.572,P=0.031),IR(r=-0.596,P=0.021),IMT was positively correlated with FPG(r=0.614,P=0.023),IR(r=0.672,P=0.004),with statistical significance(P<0.05);There was no correlation between TG and TC(P>0.05).Conclusion the abnormal metabolism of glycolipid metabolism and prehypertensive disease is cause and effect,the prevention of prehypertension and the correction of abnormal glycolipid metabolism are of important clinical significance in the prevention of hypertension.
