2005-2016年许昌市疟疾诊断与报告情况分析 被引量:3

A retrospective analysis on the diagnosis and reporting of malaria in Xuchang city of Henan province during 2005-2016

摘要 目的分析2005-2016年许昌市疟疾疫情和病例诊断与报告情况,为加强消除疟疾后监测工作、制定疟疾防治措施提供依据。方法以《传染病报告信息管理系统》中的疟疾病例为基础,描述和分析许昌市2005-2016年疟疾病例的诊断方式、不同类型疟疾病例的构成以及病例发病至确诊时间等。结果2005-2016年许昌市疟疾报告发病数分别为2、8、5、6、2、4、6、12、8、23、5、13例,共94例,其中本地病例17例,境内和境外输入病例分别为2例和75例。2011-2016年均报告发病率(0.024 6/万)比2005-2010年(0.010/万)增长146%。2005-2010年报告27例,以临床诊断为主占66.7%(18/27),未分型疟疾最多,占44.4%(12/27);2011-2016年报告67例,均为实验室确诊病例,四种类型的疟疾均有报告,恶性疟最多,占76.1%(51/67)。疟疾病例发病至确诊时间最短0 d,最长35 d,P_(50)(P_(25)~P_(75))为4(2~8)d。不同时期疟疾发病至确诊时间分布情况不同,差异有统计学意义(χ~2=9.493,P<0.01)。结论许昌市疟疾疫情有所回升,且感染类型更为复杂;病例发病至确诊时间逐渐缩短但尚未达到理想效果,应对基层医务人员加强疟防知识培训,将监测关口前移。 Objective The data of malaria epidemic, diagnosis and reporting were analyzed in order to strengthen surveillance and provide evidence for malaria control program after eliminating of malaria in Xuhang city during 2005-2016.Method Case information came from the System of Infectious Disease Reporting and Management. Detailed data, such as the diagnosis mode, the composition of different types of malaria and the diagnosis time of malaria cases were analyzed.Results The number of ases of malaria was on the rise overall in Xuchang. The average incidence rate increased by 146% in recent 6 years compared with the previous six years. Before 2011, the majority of cases were non-typed malaria, accounting for 44.4%( 12/27). After 2011, all the cases were laboratory confirmed, and all four types of malaria were reported, with the highest incidence of falciparum malaria, accounting for 76.1%(51/67). The shortest time from illness onset to diagnosis as malaria was 0 d,and the longest time was 35 d.as well the median time P50(P25-P75) was 4(2~8)d.The distribution of time from onset to diagnosis in different periods was different, and the difference was statistically significant(χ^2=9.493, P<0.01).Conclusions The outbreak of malaria in Xuchang City had risen, and the type of infection was more complicated. The time from illness onset to diagnosis reduced gradually as time goes on, but it had not achieved the desired result yet. Training for primary health care workers should be strengthened in order to move malaria surveillance gateway forward.
