广西肝细胞癌组织及癌旁中乙型肝炎病毒X基因突变体的分析

Analysis of hepatitis B virus X gene mutants in hepatocellular carcinoma tissues and cancer adjacent tissues of Guangxi

摘要 目的了解广西肝细胞癌(Hepatocellular Carcinoma,HCC)组织及癌旁中乙型肝炎病毒X基因(HBX)突变体的分布情况。方法收集2015-2016年广西医科大学附属肿瘤医院肝胆外科因HCC手术切除的HCC患者共117例,分别收集HCC组织及对应癌旁组织,巢式PCR扩增HBX基因片段并测序,用相应的生物信息学软件分析核苷酸及氨基酸序列。结果 (1)癌组织中,HBX羧基端序列缺失率和缺失长度,高于癌旁组织[(分别为22.4%和12.1%,X^2=4.868,P<0.05);(60.19±83.48)和(19.79±4.98)bp,P<0.05)];(2)癌旁组织中,插入突变仅存于血清甲胎蛋白(AFP)>20 ng/ml的患者序列中;(3)癌组织中,第88、118、119、130、131、130+131、132、143位点突变率高于癌旁组织(P<0.05);(4)K118T突变组HBV DNA水平,低于未突变组[分别为(4.50±0.96)和(4.98±1.07),P<0.05];V88N/D、D119E/N突变组ALT水平,高于未突变组[分别为(41.59±21.72)和(64.81±63.42);(41.33±22.32)和(64.36±62.75),P均<0.05];(5)差异性点突变均位于EnhII或BCP区域。结论广西HBX基因突变有多态性;HBX羧基端序列是否缺失、缺失长度及第88、118、119、130、131、130+131、132、143位点突变对广西HCC发生可能有影响。 Objective To study the distribution of hepatitis B virus X gene mutants in hepatocellular carcinoma tissues and adjacent tissues of Guangxi. Methods From 2015 to 2016, 117 patients with HCC who underwent HCC surgery in Cancer Hospital Affiliated to Guangxi Medical University were collected. The HBX gene sequences were amplified through nested PCR method and were sequenced. Sequence alignment of amino acid and nucleotide sequences were conducted by the corresponding bioinformatics software. Results 1. In cancer tissues, the loss rate and length of HBX COOH-terminal sequence were higher than of adjacent tissues(22.4% vs 12.1%, X^2=4.868, 60.19 ±83.48 vs 19.79 ±4.98, P <0.05). 2. The inserted mutation only existed in the patient sequence of AFP>20ng/ml in the adjacent tissues. 3. The mutation rate of 88,118, 119, 130, 131, 130+131, 132 and 143 sites in cancer tissues were higher than that in adjacent tissues(P<0.05). 4. The level of HBV DNA in the K118 T mutation group was lower than that the non-mutant group(P<0.05). The ALT levels in V88N/D and D119E/N mutant groups were higher than the non-mutation group(P<0.05). 5. The different mutation sites were located in the EnhII or BCP regions. Conclusion The mutation of the HBX in Guangxi was polymorphic. The deletion and deletion length of HBX COOH-terminal sequence, and the mutation of the 88, 118, 119, 130, 131, 130+131,132 and 143 sites had an effect on the occurrence of HCC in Guangxi.
