摘要
目的探讨磁共振成像(magnetic resonance imaging,MRI)定量分析甲状腺相关性眼病(thyroid associated ophthalmopathy,TAO)患者眶内组织的诊断价值。方法研究对象包括28例TAO患者(共48只眼)、18例正常对照(共36只眼)。利用MRI测量眼球突出值、眼球内侧脂肪厚度、眼外肌径线(包括短径和长径),并计算短径与长径的比值(R值)。结果TAO组眼球突出值及眼球内侧脂肪厚度均大于正常组,差异有统计学意义(P<0.05);TAO组各眼外肌的径线(短径和长径)及R值均大于正常对照组,差异有统计学意义(P<0.05);TAO组突眼度与眼球内侧脂肪厚度相关系数为0.346(P<0.05),两者间存在相关性,呈低度直线相关。结论MRI定量测量各眼外肌径线尤其是R值对于诊断TAO具有重要价值,眼球内侧脂肪厚度的测量可协助TAO的诊断。
Objective To evaluate the diagnosis of thyroid associated ophthalmopathy (TAO)in magnetic resonance imaging (MRI) by quantitative measurement of the intraorbital tissues.Methods The subjects included 28 TAO patients (48 eyes)and 18 normal controls (36 eyes).The exophthalmos value and the thickness of fatty tissue in the inner side of eyeball were measured on TIWI axial images.The short and long diameters of the extraocular muscles were measured on T2WI coronral images,and the R value (short/long diameter)was calculated.Results The TAO group showed significantly higher exophthalmos values,the thickness of fatty tissue in the inner side of eyeball,the short and long diameters of extraocular muscles and the R value than the normal group (P <0.05).The exophthalmos value is related with the thickness of fatty tissue in the TAO group (r =0.346,P <0.05).Conclusions Quantitative measurement of extraocular muscle diameters,especially the R value is useful for TAO diagnosis.Measurement of the thickness of fatty tissue can facilitate TAO diagnosis.
