双歧杆菌四联活菌片对血糖控制欠理想2型糖尿病患者肠道菌群及胰岛β细胞功能的影响 被引量:14

Influence of Bifidobacterium Tetravaccine Tables on intestinal flora and pancreatic β cell function in patients with type 2 diabetes mellitus whose blood sugar were not controlled as expected

摘要 目的探讨双歧杆菌四联活菌片对血糖控制欠理想2型糖尿病(T2DM)患者肠道菌群及胰岛β细胞功能的影响。方法选取2016年3月至2017年3月在我院内科门诊治疗的血糖控制欠理想T2DM患者94例,随机分为观察组和对照组,各47例。两组患者均予以调整饮食、适量运动及控制血糖等治疗。观察组患者加用双歧杆菌四联活菌片1.5g/次,3次/d,温水送服,连用10周。观察并记录两组患者治疗前与治疗10周后血糖控制情况、肠道菌群和β细胞功能的变化。结果治疗10周后,两组患者FBG、2hPG和HbA1C均有不同程度下降(P<0.05或P<0.01),且观察组患者变化幅度更大(P<0.05);同时两组患者肠道乳杆菌和双歧杆菌数量增加,大肠埃希菌数量减少(P<0.05和P<0.01),且观察组变化幅度更大(P<0.05);两组患者HOMA-β水平均有不同程度上升,且观察组变化幅度亦大于对照组(P<0.05)。结论血糖控制欠理想T2DM患者予以双歧杆菌四联活菌片治疗不仅可促进胰岛β细胞功能的恢复,而且能纠正肠道菌群失调,恢复肠道微生态平衡,改善糖代谢功能,有利于血糖及糖化血红蛋白的控制。 Objective To discuss the influence of Bifidobacterium Tetravaccine Tables on intestinal flora and pancreatic β cell function in patients with type 2 diabetes mellitus (T2DM) whose blood sugar were not controlled as expected. Methods 94 patients with T2DM, whose blood sugar were not controlled as expected from March 2016 to March 2017, were divided into the observation group or control group at random with 47 cases in each group. Both groups were given diet adjustment, reasonable exercise and oral hypoglycemic drugs and continuous subcutaneous insulin infusion to control blood sugar, while the observation group was additionally given Bifidobaeterium Tetravaccine Tables, 1.5 g, tid, for 10 weeks. The changes of blood sugar, intestinal flora and β cell function of patients before and 10 weeks after treatment were observed and compared between two groups. Results After treatment, the FBG, 2hPG and HbA1C of patients in both groups obviously declined (P<0.05 or P<0.01), and the changing rates in the observation group were much higher than those in control group(P<0.05). The quantities of Lactobacilli and Bifidobacteria in both groups increased than before, while that of Escherichia coli declined (P<0.05 and P<0.01), and the changing rates in observation group were much higher than those in control group (P<0.05). The HOMA-β levels in both groups obviously rose than before, and the changing rates in observation group were much higher than those in control group (P<0.05). Conclusion Bifidobacterium Tetravaccine Tables can improve the recovery of pancreatic β cell function and carbohydrate metabolism function, adjust the disorder of intestinal flora and microecological balance in the treatment of T2DM with poor control of blood sugar level, which is favorable for the control of blood sugar and glycosylated hemoglobin.
