摘要
全面医改后,药品预算管理问题成为了众多医院管理者的难题,为满足临床治疗的需求,同时解决医院临床用药成本控制问题,北京积水潭医院(以下简称我院)从总药品成本控制、临床用药精细化管理2方面进行管理。在医院药品总成本控制上,我院实施预算管理、药品动态监测和超常预警制度。对全院用药情况进行统计、测算,重点管理药品排名前列的药物,针对不同科室实际用药需求设置不同药品费用额度,并对药品使用进行排序和监测,分析药物是否合理使用,并重点关注辅助用药、特殊级抗菌药物、排名异常、增长变化异常的药物,从而有效控制药占比。对于临床用药管理,采取治疗用药优先原则,辅助用药严管措施,并对治疗用药、辅助用药、新药遴选等方面的管理进行优化。通过对药品预算管理的不断探索,我院形成了一整套科学完整的预算管理体系,在严控药品总成本预算的情况下,保证临床治疗,优化管理,有效解决了药品零差价后医院用药成本控制问题。
After the comprehensive medical reform,drug budget management has become a difficult problem for many hospital administrators.To meet the clinical treatment needs and solve the hospital clinical drug cost control problem,our hospital started a management of total drug cost control and clinical drug refinement.In terms of total drug cost control,our hospital implements budget management,drug dynamic monitoring and extraordinary early warning system. We take a number of measures:making statistics and calculation of the overall medication,focusing on the management of drugs ranked in the forefront of drugs,setting different drug cost limits for actual drug use needs of different departments,sorting and monitoring drug use,analyzing whether drugs are used rationally,and focusing on auxiliary drugs special-grade antibacterial drugs,drugs with abnormal rankings and abnormal growth changes.For the management of clinical drug use,the principle of treatment and drug use priority should be adopted. The measures for the strict control of auxiliary drug use should be taken.The management of therapeutic drug use,auxiliary drug use and new drug selection should be optimized.Through continuous exploration of drug budget management,our hospital has formed a set of scientific and complete budget management system. Under the strict control of the total cost of drugs,we ensure clinical treatment and effectively solve the problem of drug cost control.
