231株淋球菌对阿奇霉素等6种抗生素耐药观察 被引量:4

Analysis of antimicrobial resistance in 231 Neisseria gonorrhoeae

摘要 目的了解2014-2016年德清地区231株淋球菌对阿奇霉素等6种抗生素的耐药性的变化,为临床治疗淋病提供理论依据。方法采用世界卫生组织推荐的琼脂稀释法检测淋球菌对5种抗菌药物的敏感性和质粒介导高度耐四环素的淋球菌(TRNG)菌株;用纸片酸度定量法测定产β-内酰胺酶淋球菌(PPNG)菌株。结果231株菌株中检测出PPNG113株(48.92%),TRNG125株(54.11%),青霉素耐药207株(89.61%);大观霉素和头孢曲松敏感性较好为100.00%,未发现耐药株,但头孢曲松中敏菌株有13株(5.63%),阿奇霉素耐药菌株有51株(22.08%),环丙沙星的耐药率最高,达到了100.00%。结论淋球菌对于环丙沙星、四环素和青霉素的耐药性已处于较高水平,已经不适宜用于治疗淋病。而大观霉素、头孢曲松的敏感性较好,可作为首选药,其次是阿奇霉素,但其敏感性的减弱,说明实时监测其耐药性的变化仍然十分重要。 Objective To understand the changes of 231 strains antimicrobial resistance in neisseria gonorrhoeae from 2014 to 2016 and provide basis for clinical treatment of gonorrhea.Methods Sensitivity of neisseria gonorrhoeae to 5 antibacterial agents and high level tetracycline resistant neisseria gonorrhoeae(TPNG)were determined by AGAR dilution method recommended by World Health Organization(WHO)and β-lactamase by paper iodine quantitative method.Results 112(48.92%)strains were PPNG and 125(54.11%)strains were TRNG in 231 neisseria gonorrhoeae.231 strains(81.9%)showed resistance to Penicillin.The sensitivity of spectinomycin and ceftriaxone were better(100%)and no resistant strain was found.However,13 strains(5.63%)showed reduced sensitivity to ceftriaxone;51 strains of azithromycin resistant strain(22.08%)and ciprofloxacin had the highest tolerance rate(100%).Conclusion Gonococcal drug resistance to ciprofloxacin,tetracycline and Penicillin is at a higher level and not suitable for gonorrhea.But the sensitivity of spectinomycin and ceftriaxone is better and can be used as the first choice to treat gonorrhea.The second choice is azithromycin.Its sensitivity weakens,indicating that it is important to monitor the changes in its drug resistance.
