摘要
目的分析超声造影诊断肝脓肿患者感染的临床价值。方法选取医院2015年1月-2017年8月收治的80例肝脓肿患者作为研究对象。回顾分析肝脓肿患者的常规超声与超声造影的表现。观察患者病原菌分布、病灶诊断准确率以及声像图的差异。结果肝脓肿患者男性、在60岁以上居多,病灶多呈单发性且多出现于脓肿形成期,临床多表现腹痛及发热,常出现胆囊炎等并发症。治疗前后,病原菌分布无显著变化。经超声造影分析,患者在脓肿炎性期、脓肿形成初期及脓肿形成期的诊断率明显高于经常规超声诊断,两组差异具有统计学意义(P<0.05)。超声造影:41个病灶平均最大截面积为(3.06±1.27)cm^2,常规超声:41个病灶平均最大截面积为(46.73±12.31)cm^2,两种差异具有统计学意义(t=5.119,P=0.017)。脓肿炎性期、脓肿形成初期及脓肿形成期的影像学表现变化明显,具有统计学意义(P<0.05)。结论超声造影能有效提高肝脓肿患者的诊断准确性,其影像学在肝脓肿不同阶段具有不同的特点,在临床中值得应用推广。
OBJECTIVE To study the clinical effect of contrast-enhanced ultrasonography in diagnosis of patients with liver abscess complicated with infection.METHODS 80 cases of patients with liver abscess admitted to the hospital during Jan.2015 to Aug.2017 were enrolled in the study.Behaviors of conventional ultrasonography and contrast-enhanced ultrasonography of these patients were retrospectively analyzed.The differences in pathogen distribution,diagnostic accuracy of the lesion and sonographic images were observed.RESULTS Most of the liver abscess patients were male and over 60 years old.Most of the lesions were solitary and occurred in the formation stage of abscess.The clinical manifestations were abdominal pain and fever,and often complicated with cholecystitis and other complications.There was no significant change in the distribution of pathogens before and after treatment.The diagnostic rates of abscess inflammatory stage,abscess formation stage and abscess formation stage by contrast-enhanced ultrasonography were significantly higher than conventional ultrasonography,and the difference(P<0.05).Contrast-enhanced ultrasonography revealed an average maximum cross-sectional area of the 41 lesions of(3.06±1.27)cm^2,whereas the value obtained by conventional ultrasonography was(46.73±12.31)cm^2,the difference was significant(t=5.119,P =0.017).The imaging manifestations of the abscess inflammatory stage,the initial stage of abscess formation and the stage of abscess formation changed significantly(P<0.05).CONCLUSION Contrast-enhanced ultrasonography can effectively improve the diagnostic accuracy of patients with liver abscess,and its imaging has different characteristics in different stages of liver abscess.It is worth promoting in clinic.
