摘要
目的 探讨 Oddi括约肌松弛与返流性胆管炎和结石复发的关系。方法 回顾性研究经手术证实 Oddi括约肌松弛 ,乳头通过 1cm以上的胆管探子 ,术后仍反复胆管炎和 /或复发结石 5 3例的病理与治疗。结果 再次手术行横断胆总管的胆管空肠 Y型吻合术 (2 9例 )或胃改道术 (12例 )等术式 ,基本消除或明显减轻胆管返流因素效果良好。单纯再次胆管探查取石 T管引流和非手术治疗效果不良。结论 Oddi括约肌松弛可以导致复发性返流性胆管炎和 /或结石复发 。
Objevtive To discuss the relationship between Oddi's sphincter relaxation and recurrent inflammation of biliary ducts and the recurrence of biliary calculus.Methods 53 cases of biliary calculus with Oddi's sphincter relaxation proved by surgery were reviewed.These patients suffered from the recurrent inflammation of biliary duct and/or recurrent biliary calculus after operation.The pathologic results and treatment method of these 53 patients were discussed.Results 29 patients of the 53 cases underwent cholangiojejunostomy,12 patients did Roux-Y type gastrojejunostomy.The reflux of biliry ducts was cured after the opration.Other patients who underwent exploration of biliary tract of hepatocholangiostomy or conservative treatment had poor effect.Conclusion Oddi's sphincter relaxation could lead to recurrent inflammation of bliary duct and /or the biliary calculus.Eliminating the reflux of biliary ducts by surgery would have good effect.
