摘要
[目的]观察消积饮(由黄芪、云芝、莪术、白花蛇舌草等组成)及其与化疗结合对中晚期肺癌的治疗作用。[方法]将112例中晚期肺癌患者随机分为消积饮组、化疗组和消积饮加化疗组3组。消积饮组予以消积饮治疗;化疗组根据肿瘤的病理类型,鳞癌用CAP方案,腺癌用EP方案,小细胞肺癌用CE方案;消积饮加化疗组同时予消积饮加化疗药治疗。各组均治疗1个疗程,比较各组临床症状、生存质量、癌灶大小、远处转移、T淋巴细胞亚群的变化以及药物不良反应和生存期的情况。[结果]在对瘤体治疗方面,消积饮组对肺癌的有效率为4.08%,稳定率为53.06%,化疗组为21.21%、48.48%,消积饮加化疗组为46.67%、76.67%;消积饮组的缩瘤效果较化疗组差(P<0.05);消积饮组的远处转移率明显低于化疗组(分别为19.05%、69.23%,P<0.01);消积饮组的中位生存期长于化疗组(375d、234 d,P<0.01);消积饮组生存质量提高和稳定的患者多于化疗组(P<0.01);消积饮组治疗后T淋巴细胞亚群中CD+3、CD+4的比例较治疗前显著升高(P<0.01),CD+8的比例显著降低(P<0.01)。消积饮组无明显不良反应,对心、肝、肾等器官无损伤作用;消积饮加化疗组抑制肺癌的作用优于消积饮组和化疗组;而不良反应较化疗组轻(P<0.01)。[结论]消积饮能够缓解中晚期肺癌患者临床症状,提高患者细?
ive] To observe the therapeutic effects of Xiaoji Decoction (XD) in treating intermed iate and late stages of lung cancer. [ Methods] One hundred and twelve patients with lung cancer in stage ? -? were randomly allocated to Group A, Group B and Group C. Group A ( n = 49) was treated with XD alone, Group B ( n = 33 ) was treated with various chemotherapeutic regimens according to the histo-logical types of lung cancer: CAP regimen for lung squamous carcinoma, EP regimen for lung adenocarci-noma and CE regimen for small cell carcinoma of lung, In Group C, 30 patients were treated with chemo therapy combined with XD. Symptomatic relief, life quality, tumor size, distant metastasis, sub-types of T lymphocytes, survival period and side effects in the three groups were evaluated after one course of treatment. [Results] The effects of XD on tumor size were evaluated by the effective rate and stabilized rate. They were 4.08%and 53.06%, 21.21% and 48.48%, and 46.67%and 76.67%in Group A, Group B and Group C respectively. The differences between Group A and Group B were significant ( P<0.05). As compared with Group B, distant metastatic rate was decreased in Group A (19.05% vs 69.23%), the median survival period prolonged (375 days vs 234 days) and We quality increased ( P< 0.01) . The percentages of CD3 + , CD4+ of T lymphocyte subcategory were increased and the percent age of CD+8 decreased in Group A after treatment as compared with those before treatment ( P < 0.01) . No obvious side effects on heart, liver and kidney were found in Group A. [Conclusion] XD can alleviate the symptoms, increase cellular immune function, counteract the metastasis of lung cancer, improve the quality of life and increase the tumor-bearing survival time of patients in intermediate and late stages. XD combined with chemotherapeutic agents can increase the tumor-inhibitory rate and alleviate the side effects of chemotherapeutic agents. The mechanism of XD for intermediate or late stages of lung cancer may be associated with the increase of cellular immune function.
