Losartan降压效应及对高血压病患者血浆肾素、血管紧张素Ⅱ、醛固酮水平的影响

摘要 目的 探讨 Ang 受体拮抗剂 L osartan对 EH患者的降压效应及血浆 RAAS的影响。方法  1~ 3级高血压病患者 6 5例 ,给予 L osartan 5 0 mg/d治疗 8周 ,分别于治疗前、后测量患者血压、心率。并分别于治疗前、1周、8周时用放射免疫法测定血浆 PRA、Ang 、 Ald活性。结果  1L osartan对 1~ 3级高血压患者 SBP、DBP均有下降作用 ,降压同时心率不增快。 2 L osartan治疗 1周时血浆 PRA、Ang 显著高于治疗前 (P<0 .0 5或 P<0 .0 1)。 Ald则较治疗前降低 (P<0 .0 5 )。 8周时 PRA、Ang 仅轻度升高与治疗前相比差异无显著性 (P>0 .0 5 ) ,Ald则较 1周时有进一步下降 ,但无显著性 (P>0 .0 5 )。结论  1L osartan 5 0 mg/d能有效降低不同程度的 SBP、DBP,降压同时心率无反射性增加。 2长期服用 L osartan不会引起血浆 PRA、Ang 持久异常增高 ,而 Objective To investigate the effects of angiotensin Ⅱ receptor antagonist Losartan on the blood pressure and the plasma renin angiotensin aldosterone system in patients with essential hypertension.Methods Sixty five patients with grade one to three essential hypertension were included in this study.Losartan 50mg/d were administered in each of these patients,and SBP,DBP and HR were detected before and after treatment.Plasma PRA,AngⅡ and Ald were determined with RIA before treatment and at 1 and 8 weeks in sixty of these patients.Results Losartan lowered the blood pressure(SBP and DBP)obviously in essential hypertensive patients( P <0.05 and P <0.01)while it did not increase the heart rate in these patients.After one week of Losartan treatment,the plasma PRA and AngⅡ increased obviously( P <0.05 and P <0.01,vs before treatment)while the plasma Ald level decreased significantly( P <0.05,vs before treatment).At 8 weeks,the plasma PRA and AngⅡ levels were slightly higher than those before treatment ( P >0.05) while the Ald level was further decreased in comparison with that at one week,though not,significantly.Conclusion Angiotensin Ⅱ receptor antagonist Losartan is an effective drug for lowering the blood pressure without increasing heart rate in patients with essential hypertension.The long term use of Losartan can decrease the plasma Ald level obviously and will not induce persistant increase in PRA and AngⅡ.
