摘要
目的 :为探讨截石位对手术后并发症的影响 ,对传统截石位的摆放方法进行了改良。方法 :选择截石位手术成年病人140例 ,随机分为两组 ,传统方法组将托腿板支托病人大腿 ,托腿板关节端置于窝处 ;改良方法将托腿板从支托大腿改成支托小腿 ,避开对窝的压迫 ,从而防止托板压迫血管和腓总神经。结果 :传统组手术中踝部大隐静脉压力明显大于改良组 (P<0 .0 1) ;术后传统组病人下肢疼痛麻木等不良反应发生率高于改良组 ;传统组有 1例发生腓总神经损伤。结论 :改良摆放方法有利于减少截石位术后的并发症。
Objetive:To investigate the effects of lithotomy position on postoperative complications. Methods:140 patients who were scheduled for lithotomy surgery were randomly divided into a traditional group and a modified group. In the traditional group,the leg raising plate supported the popliteal fossa and the femoral part. In the modified group,the leg raising plate supported the shank instead of the femoral part.This may prevent oppression of the popliteal fossa and injury of the peroneal nerve and the popliteal blood vessels. Results: During the operation ,the pressure of the great saphenous vein in the traditional group was significantly higher than that of the modified group( P <0.01). After surgery,the incidence of side effects in the traditional group was higher than that of the modified group.One patient had peroneal nerve injury in the traditional group. Conclusion: The modified lithotomy position may reduce the incidence of postoperative complications. Author's address Liaocheng People's Hospital,Shandong Province,China 252000
