摘要
目的通过研究在临床中使用持续性区域动脉灌注(continuous regional artery infusion,CRAI)治疗重症急性胰腺炎(SAP),分析CRAI治疗SAP患者的疗效。方法将60例SAP患者随机分为治疗组和对照组。两组患者均采用治疗原则相同的西医基础综合治疗方法。治疗组30例予CRAI治疗,对照组30例用传统外周静脉给药途径进行治疗。观察治疗前及治疗7d后患者临床症状、体征,生化指标变化情况及总临床疗效。结果 CRAI能缩短SAP患者临床腹痛、腹胀时间,有效改善SAP患者的血尿淀粉酶、血钙、WBC水平,提高治疗SAP患者的临床疗效,减少并发症。结论CRAI可以提高SAP的临床疗效,减少并发症,值得临床推广。其作用途径可能与改善胰腺微循环、提高药物局部血药浓度有关。
Objective To analyze efficacy of continuous regional artery infusion （CRAI） for the treatment of patients with severe acute pancreatitis （SAP）. Methods Sixty patients with SAP were randomly divided into a treated group and a control group. Patients in both groups were given the basic western comprehensive treatment based on the same principles. Thirty cases in treated group were given CRAI treatment, while 30 patients in the control group were treated with the traditional peripheral vein drug administration. The patients＇ clinical symptoms, signs, biochemical indexes and the changes of the total clinical curative effect were observed before treatment and 7 days after treatment. Results CRAI could shorten the duration of clinical abdominal pain and abdominal distension and effectively improve the urine amylase, blood amylase, blood calcium and WBC level in patients with SAP, as well as raise the clinical curative effect, and reduce the complications. Conclusion CRPI can improve the clinical curative effect of SAP, reduce the complications, and is worthy of clinically recommended. Its mechanism may be correlated with improving the pancreas microcirculation and lo cal blood drug concentration.
