摘要
目的分析探讨原发性高血压血压昼夜节律异常与肾损害的关系。方法选择2012年6月。2014年2月我院收治并行24h动态血压监测的原发性高血压患者154例,根据昼夜血压监测结果将其分为杓型组51例、非杓型组64例、反杓型组32例和深杓型组7例。观察比较各组患者血压昼夜节律异常情况,并对所有研究对象的尿微量白蛋白(mALB)、血尿素氮(BUN)、血肌酐(SCr)和血尿酸(UA)进行检测。结果4组患者日间血压比较无明显差异(P〉0.05),非杓型、反杓型和深杓型组患者夜问舒张压、收缩压均明显高于杓型组,反杓型组患者的24h脉压差也明显高于杓型组(P〈0.05)。4组患者在尿微量白蛋白、血尿素氮和血肌酐上有显著差异(P〈0.05),而在血尿酸上的差异则无统计学意义(P〉0.05)。结论原发性高血压血压昼夜节律异常与肾损害有关.非杓型、反杓型和深杓型高血压患者有发生早期肾损害的倾向。
blood pressure Objective To investigate the relationship between the abnormal circadian rhythm of and the renal damage in patients with primary hypertension. Methods A total of 154 patients with primary hypertension who were treated and underwent 24-h ambulatory blood pressure monitoring in our hospital from June 2012 to February 2014 were enrolled in this study. They were divided into four groups with 51 in dipper group, 64 in non-dipper group, 32 in anti-dipper group and 7 in deep-dipper group. The abnormal circadian rhythm of blood pressure was observed and compared among the groups. The urinary micro albumin (mALB), blood urea nitrogen (BUN), serum creatinine (SCr) and uric acid (UA) were detected in all patients. Results There was no sig- nificant difference in the daytime blood pressure among the four groups ( P 〉 0. 05 ) . The nocturnal systolic and diastolic blood pressure was significantly higher in non-dipper group, anti-dipper group and deep-dipper group than in dipper group (P 〈 0. 05 ) . The 24-h pulse pressure was also significantly increased in anti-dipper group when compared with deep-dipper group (P 〈 0. 05 ) . Significant difference was noted in the mALB, BUN and SCr except UA among the four groups (P 〈 0. 05 ) . Conclusion The abnormal circadian rhythm of blood pressure is correlated with the renal damage in patients with primary hypertension. Patients with a non-dipper, anti-dipper or deep-dipper pressure profile are more likely to develop early renal damage.
