不同程度皮质性白内障对多焦视网膜电流图结果评判影响 被引量:5

The effects of cortical cataract severity on multifocal electroretinogram

摘要 目的研究不同程度皮质性白内障对多焦视网膜电流图(muhifocal electroretinogram,mERG)结果评判的影响。方法临床病例对照研究。对2012年11月1日至2013年3月31日在解放军第180医院眼科住院行白内障手术的皮质性白内障患者40例,按照白内障混浊程度LOCS111分级,分为A组(28例)双眼白内障轻重程度明显不一,1只眼C1或C2(轻度白内障眼);另1只眼C5(重度白内障眼);B组(12例)1只为人工晶状体眼(IOL眼),另1只眼C5(重度白内障眼)。所有病例均排除白内障以外的眼病及全身病。应用德国罗兰公司的mERG仪记录各环N1波、P1波的振幅密度和潜伏期,比较A、B组患者双眼之间的差别。结果A组重度白内障眼的N1波1环、2环和3环,P1波1环和2环的振幅密度较轻度白内障眼低,差异有统计学意义,P〈0.05(N1波t1环=2.499,P=0.019;t2环=2.556,P=0.017;t3环=-2.960,P=0.006;t1环=-6.871,P=0.001;t2环=-3.259,P=0.003)。B组重度白内障眼的P1波I环和2环的振幅密度较IOL眼低,差异有统计学意义,P〈0.05(P1波t1环=-3.014,P=0.012;t2环=-3.298,P=0.007)。A、B组其余各环的振幅密度和1~5环的潜伏期,双眼比较差异均无统计学意义(P〉0.05)。A组重度自内障眼相对轻度白内障眼P1波1环的振幅密度下降(25.7±18.0)%。B组重度白内障眼相对IOL眼P1波1环的振幅密度下降(24.7±27.5)%。A组轻度白内障眼与B组IOL眼相比,N1波和P1波各环的振幅密度和潜伏期差异均无统计学意义(P〉0.05)。结论成熟期或过熟期皮质性白内障会导致多焦视网膜电流图P1波振幅下降而潜伏期不受影响,对于重度皮质性白内障术前多焦视网膜电流图结果的判断需考虑晶状体混浊程度的影响。 Objective To explore the effects of cortical cataract severity on multifocal electroret- inogram (mfERG). Methods Forty subjects with cortical cataracts were enrolled in the study and lens opacity was classified according to LOCSIII. Patients in group A (28 cases) had C1 or C2 cata-racts in one eye (eyes with mild cataracts) and C5 in the other eye (eyes with severe cataracts); pa-tients in group B (12 cases) had already taken cataract surgery in one eye (IOL eyes) and had C5 cataracts in the other eye (eyes with severe cataracts). No subjects had any significant eye disease except for cataracts. All cases underwent examination of mfERG using RETI scan produced by Ro-land in Germany. Both the amplitudes and the latencies of N1 and P1 of first-order responses in two groups were used for analysis.
