摘要
目的探讨芪苈强心胶囊辅治慢性肾功能不全(CRF)伴慢性心力衰竭(CHF)患者的疗效及应用价值。方法选取CRF伴CHF患者60例,随机分为观察组与对照组各30例,2组均给予常规治疗,观察组另加服芪苈强心胶囊,疗程12周,观察患者治疗前后血肌酐(SCr)、尿素氮(BUN)、血浆氨基末端脑钠肽前体(NT-proBNP)、24 h尿蛋白、左心室射血分数(LVEF)、左心室舒张末内径(LVEDD)、心输出量(CO)及中医症候变化。结果治疗后2组SCr、BUN、NT-proBNP、24 h尿蛋白均较治疗前下降(P<0.05),观察组较对照组下降更为明显(P<0.05);LVEF、CO治疗后均较治疗前上升,LVEDD均下降(P<0.05),观察组较对照组改善更为明显(P<0.05);观察组治疗总有效率为76.67%,高于对照组的46.63%(P<0.05);地高辛使用量观察组平均(0.114±0.053)mg/d,少于对照组的(0.235±0.068)mg/d(P<0.05)。结论芪苈强心胶囊辅治CRF伴CHF,能明显改善患者肾功能及心功能,减少洋地黄制剂使用剂量,用药安全。
Objective To explore the Qiliqiangxin capsule for the treatment of chronic renal insufficiency （ CRF） pa-tients with congestive heart failure （ CHF） with curative effect and application value .Methods Select 60 cases of patients with CHF with CRF, patients were randomly divided into observation group and control group （30 cases in each group）, two groups were given conventional treatment , the observation group add Qiliqiangxin capsule orally, after 12 weeks of treatment, observation of patients before and after treatment serum creatinine （SCr）, urea nitrogen （BUN）, plasma amino terminal brain natriuretic peptide precursor （NT-proBNP）, 24 h urine protein and left ventricular ejection fraction （LVEF）, left ventricular end diastolic diameter （LVEDD）, cardiac output （CO） change, TCM symptoms change.Results Two groups after treatment SCr, BUN, NT-proBNP, 24 h urinary protein decreased than before treatment （ P 〈0.05）, the observation group decreased more significantly than in the control group （ P 〈0.05）; after treatment LVEF, CO were raised than before treatment , LVEDD decreased （ P 〈0.05）, the observation group improved more significantly than the control group （ P 〈0.05）;ob-servation group total effective rate were 76.67%, higher than that of control group of 46.63%（ P 〈0.05）;digoxin average amount of observation group （0.114 ±0.053） mg/d, less than the control group （0.235 ±0.068） mg/d （ P 〈0.05）. Conclusion Qiliqiangxin capsule assisted treatment CRF with CHF , can significantly improve cardiac function in patients with renal function and reduce the dose of digitalis preparations .
