肿瘤坏死因子-α基因启动子-238位点基因多态性与强直性脊柱炎的相关性Meta分析 被引量:3

Association of polymorphism in-238 site of TNF-α promoter region with ankylosing spondylitis: a meta-analysis update

摘要 目的进一步探讨肿瘤坏死因子(tumor necrosis factor,TNF)-α基因启动子-238位点多态性与强直性脊柱炎(ankylosing spondylitis,AS)的相关情况。方法通过中国生物医学文献数据库、中国期刊全文数据库、万方、维普、PubMed、Cochrane Library、Ovid、Wiley Online Library、EBSCO、Elsevier Science Direct、Springer、Goolgescholar数据库检索国内外已发表或未发表TNF—α基因启动子一238位点多态性与AS相关的病例对照研究,采用Meta分析综合定量分析最终纳入的文献。结果17篇文献共纳入1899例患者和2381例对照,Meta分析结果提示在总人群(OR=0.96,95%CI:0.76一1.21,P=0.720)、亚洲人群(OR=1.38,95%6/:0.84~2.24。P=0.200)、非亚洲人群(OR=0.97,95%6/:0.61—1.55,P=0.900)均不能明确TNF—Ot-238位点(GA十AA)vs.GG基因型与AS间存在关联;非亚洲人群(OR:0.40,95%CI:0.08~2.03,P:0.270)均不能明确TNF-α-238位点(GG+GA)VS.AA基因型与AS间存在关联;在总人群(OR:0.95,95%6/:0.52~1.74,P=0.870)、亚洲人群(OR=1.39,95%6/:0.90~2.14,P=0.140)、非亚洲人群(OR=0.79,95%6/:0.33~1.86,P=0.590)均不能明确TNF—α-238位点GVS.A等位基因多态性与AS间存在关联。结论Meta分析显示总人群、亚洲人群和非亚洲人群TNF—α-238位点各基因型和等位基因多态性与AS间均未显示关联。 Objective To further explore the association of the polymorphism in -238 site of tumor necrosis factor (TNF)-α promoter region with ankylosing spondylitis(AS). Methods The references between polymorphism in -238 site of TNF-α promoter region with AS were retrieved through Chinese Biomedical Database ( CBM ), Chinese National Knowl- edge Infrastructure (CNKI) , Wan Fang, Wei Pu, PubMed, Cochrane Library, Ovid, Wiley Online Library, EBSCO, Elsevier Science Direct, Springer, Goolge scholar databases, until September 20, 2013. A recta-analysis was used to systematically analyze the literatures that were included finally. Pooled odds ratios (OR) with 95% confidence intervals (CI) were appropriately derived from random-effects models or fixed-effects models. Results 17 studies were included in the fi- nal meta-analysis, enrolling a total of 1 899 AS cases and 2 381 controls. Subgroup ( Asian and non- Asian) analysis were stratified by ethnieity. The recta-analysis showed that the frequencies of ( GA + AA ) vs. GG genotypes had no statistical difference between AS group and the control group in the global group ( OR =0. 96, 95% CI: 0. 76-1.21, P =0. 720), in the Asian populations ( OR = 1.38, 95% CI: 0. 84-2. 24, P = 0. 200) and in the non-Asian populations ( OR = 0. 97, 95 % CI: 0. 61-1.55, P = 0. 900). The analysis also showed that the frequencies of ( GG + GA) vs. AA genotypes had no statistical difference between AS group and the control group in the global group ( OR =0.40, 95% CI: 0. 08-2. 03, P = 0. 270) and in the non-Asian populations (OR =0. 40, 95% CI: 0. 08-2. 03, P =0. 270). The analysis also showed that the frequencies of G vs. A alleles had no statistical difference between AS group and the control group in the global group (0R=0.95, 95% CI: 0.52-1.74, P =0.870), in the Asian populations (OR = 1.39, 95% CI: 0.90-2. 14, P = 0. 140) and in the non-Asian populations ( OR = 0. 79, 95% CI: 0. 33-1.86, P = 0. 590). Conclusions The meta-anal- ysis revealed that the TNF-ct -238 G/A polymorphisms may not be associated with AS. However, the meta-analysis needs to be confirmed by more studies with larger population of various ethnicity and stratified by clinical characteristics in the future.
