摘要
目的探讨会阴无保护接生技术在低危初产妇分娩中的应用效果。方法低危初产妇按随机数字表法将其分为观察组和对照组。观察组192例采用会阴无保护接生技术，对照组185例采用常规接生技术。比较两组产妇会阴裂伤发生率、第二产程时间及产程中疼痛情况，产后半年分别对这些产妇进行电话随访，了解性生活恢复情况。结果观察组产妇的观察组会阴撕裂程度轻于对照组，比较差异具有统计学意义（z=-8．28，P〈0．05）；两组产妇第二产程时间的差异无统计学意义（t=-0．81，P〉0．05），产后出血量的差异有统计学意义（t=3．47，P〈0．05）；观察组产妇产程疼痛级别低于对照组，差异有统计学意义（z=-4．39，P〈0．05）；产后随访观察组开始性生活时间早（X^2=1．73，P〈0．05），发生性生活疼痛的比率较对照组降低，差异有统计学意义（t=7．11，P〈0．05）。结论会阴无保护接生可降低会阴侧切率，减少产后出血，提高分娩舒适感，提高产后性生活质量，促进自然分娩率，值得临床推广应用。
Objective To study the effect of unprotected perineum delivery technique in spontaneous vaginal delivery of low risk gravida. Methods Gravidas were divided into observation group （n = 192 ） and control group （n = 185 ） by random number table. Unprotected perineum delivery technique and routine delivery technique were adopted in the observation and control group respectively. The incidence of perineal laceration, time of second stage of labor, and labor pain in two groups were compared. Their sexual life recuperation was analyzed with telephone followed-up half year after delivery. Results The severity of perineal laceration of the observation group was significantly milder than the control group （z = - 8.28, P 〈 0.05 ）. No significant difference was detected in the time of the second stage of labor between two groups （t = -0.81 ,P 〉 0.05 ） , but there was difference in the amount of bleeding after delivery between two groups （t = 3.47 ,P 〈 0.05 ）. The labor pain level of the observation group was significantly lower than that of the control group （z = -4.39, P 〈 0.05 ）. The time of sexual life recuperation after delivery was earlier in the observation group （X^2 = 1.73, P 〈 0.05 ） , and the rate of sexual pain was lower （ t = 7.11, P 〈 0.05 ）. Conclusion Unprotected perineum delivery technique can decrease lateral episiotomy rate, reduce bleeding amount, improve comfort feeling at delivery and sexual life quality and promote natural deliver rate. It deverses further clinical application.
