摘要
目的:研究瑞舒伐他汀钙对冠状动脉造影术后冠心病患者的肾功能保护作用,并探讨其作用机制。方法:选择行冠状动脉造影术的182例患者随机分为观察组与对照组。2组患者仅观察组患者术前3 d开始服用瑞舒伐他汀钙40 mg·d-1。比较2组患者手术前后肾脏功能及血清炎性细胞因子水平。结果:术前2组患者血肌酐(Scr)、血清胱抑素(Cys-c)、β2-微球蛋白(β2-MG)和脂质运载蛋白(NGAL)水平差异无统计学意义(P>0.05),术后第1天、第2天和术后第5天观察组患者(CysC、β2-MG和NGAL水平均显著低于对照组(P<0.01),但是Scr水平仅对照组术后第2天显著低于对照组(P<0.01)。术后对照组和观察组分别有5例(5.49%)和0例(0%)患者出现造影剂肾病(CIN),差异具有统计学意义(P<0.05)。术前2组患者高敏C-反应蛋白(Hs-CRP)、白介素-6(IL-6)和肿瘤坏死因子(TNF-α)水平差异无统计学意义(P>0.05),但是术后第1天、第2天和术后第5天观察组患者上述炎性细胞因子水平均显著低于对照组(P<0.01)。结论:瑞舒伐他汀钙早期干预对冠心病患者行冠状动脉造影术患者的肾功能具有保护作用,其机制可能与减轻造影剂导致的炎症反应有关。
OBJECTIVE To investigate the renal protective effect of rosuvastatin calcium on renal damage induced by contrast agent of angiography and to explore its mechanism of actions. METHODS 182 patients with coronary heart disease received angiography were randomly divided into observation group and control group. Cases in observation group were given with rosuvastatin calcium 40 mg. d^- 1. The indicators of renal function and the level of serum inflammatory cytokines were compared before and after operation. RESULTS The levels of Scr, Cys-c, β2-MG and NGAL in observation group were not significantly different compared with those in control group before operation (P〉0. 05). The levels of Cys-c, β2-MG and NGAL in observation group were significantly lower than control group in 1st, 2nd and 5th day after operation (P〈0. 01), and the level of Scr were significantly lower than control group in 2nd day after operation (P〈0. 01). There were respectively 5 cases (5.49%) and no case (0%) with CIN after operation with difference in statistic (P〈0. 05). The levels of Hs-CRP, IL-6 and TNF-a in observation group were not significantly different compared with those in control group before operation (P〉0. 05). The levels of Hs-CRP, IL-6 and TNF-a in observation group were significantly lower than control group in 1st, 2nd and 5th day after operation (P〈0. 01). CONCLUSION Rosuvastatin calcium has the protective effect on renal damage induced by con- trast agent of angiography, which is probably related to the decrease in the inflammatory reaction induced by contrast agent.
