摘要
目的:探讨妊娠糖尿病(GDM)患者血清同型半胱氨酸(Hcy)、胱抑素C(CysC)、CRP水平的变化及其与胰岛功能的关系。方法:选择2012年1-12月笔者所在医院门诊确诊的240例GDM患者为观察组,100例同期健康孕妇作为对照组。通过对血Hcy、CysC、CRP、空腹血糖(FBG)、空腹胰岛素(FINS)的测定,比较两组患者血Hcy、CysC、CRP、FBG、FINS、HOMA-IR、HOMA-β水平,并分析血Hcy、CysC、CRP与HOMAIR、HOMA-β的相关性。结果:观察组Hcy、CysC、CRP、FBG、FINS、HOMA-IR水平高于对照组(P<0.05)。两组间HOMA-β比较,差异无统计学意义(P>0.05)。Hcy、CysC、CRP与HOMA-IR正相关(P<0.05),与HOMA-β无明显相关性(P>0.05)。结论:GDM患者HOMA-IR、FINS、CRP、Hcy、CysC明显升高,Hcy、CysC、CRP与HOMA-IR正相关。GDM患者存在胰岛素抵抗及低度炎症反应,炎症因子与胰岛素抵抗密切相关,炎症因子参与了GDM的发生、发展,血Hcy、CysC、CRP水平可作为GDM患者的监测指标。
Objective：To explore the changes of homocysteine（Hcy）,cystatin C（CysC） and CRP level in the serum of gestational diabetes mellitus patients（GDM）,and the relationship between the changes of that and islet function.Method：A total of 240 cases with GDM diagnosed by outpatient from January to December in 2012 were selected as an observation group,and 100 healthy pregnant women in the same period as a control group were selected.Levels of Hcy, CysC,CRP,FBG,FINS,HOMA-IR and HOMA-βin the blood between these two groups were compared by determining Hcy,CysC,CRP,fasting blood glucose（FBG） and fasting insulin （FINS） in the blood.Then the correlation between Hcy,CysC,CRP and HOMA-IR,HOMA-βwere analyzed.Result：The levels of Hcy,CysC,CRP,FBG,FINS and HOMA-IR in the observation group were significantly higher than that in the control group（P〈0.05）.HOMA-β’s level between the two groups was compared,and the difference was not statistically significant（P〉0.05）.The levels of Hcy,CysC and CRP are positively correlated with HOMA-IR’s level（P〈0.05）,but there is no significant correlation between HOMA-β’s level and levels of Hcy,CysC and CRP （P〉0.05）.Conclusion：The levels of Hcy,CysC,CRP,FINS and HOMA-IR of GDM patients were significantly increased,and the levels of Hcy,CysC and CRP are positively correlated with HOMA-IR’s level.GDM patients have insulin resistance and low-grade inflammation.Inflammatory factors and insulin resistance are closely related.Inflammatory factors are involved in the occurrence and development of GDM.The determination of the levels of Hcy,CysC and CRP in the blood can be the monitoring indicators of patients with GDM.
