摘要
目的探讨应用复方南星止痛膏配合经皮激光椎间盘减压术（PLDD）治疗椎间盘源性腰痛的临床疗效。方法将山东省聊城市第二人民医院2007年10月至2011年4月收治的98例椎间盘源性腰痛患者按照随机数字表法分为治疗组和对照组，每组各49例。治疗组行PLDD治疗，术后应用复方南星止痛膏；对照组行PLDD治疗，术后应用扶他林。比较2组患者的疗效、视觉模拟评分（VAS）、腰椎功能障碍指数（ODI）评分及并发症情况。结果术后1周治疗组有效率明显高于对照组[93．8％（46／49）比55．1％（27／49）]，差异有统计学意义（P〈0．01）；术后1、3、6个月2组有效率比较差异无统计学意义（P〉0．05）。2组患者治疗后VAS与ODI评分与治疗前比较[治疗组：（3．0±1．9）分比（6．4±1．2）分，（17．5±7．5）分比（47．1±8．3）分；对照组：（4．9±1．7）分比（6．4±2．8）分，（23．6±10．1）分比（47．0±7．9）分]，差异有统计学意义（P〈0．01）。治疗组治疗后VAS与ODI评分与对照组比较差异有统计学意义（P〈0．01）。治疗组并发症发生率[（18．4％（9／49）]明显低于对照组[61．2％（30／49）]，差异有统计学意义（P〈0．01）。结论应用复方南星止痛膏配合PLDD可明显提高患者的治疗有效率，改善疼痛状况及腰椎功能，且不良反应发生率低。
Objective To study the therapeutic effect of Fufang nanxing zhitong plaster cooperated with percutaneous laser disc decompression （PLDD） in the treatment of discogenie back pain. Methods Ninety-eight eases of discogenic back pain in the Affiliated second people＇s hospital of Liaoeheng from October 2007 to April 2011 were divided into 2 groups by means of random number table： treatment group and control group with 49 cases in each group. The treatment group was given Fufang nanxing zhitong plaster after PLDD; the control group was given diclofenac sodium sustained release tablets after PLDD. The curative effect, visual analogue scale （VAS） score, ODI score and complications situation were compared. Results 1 week after operation, the excellent and good rate of the treating group was significantly higher than that of the control group [ 93.8% （46/49） vs 55.1% （27/49）]; there were significant differences between the two groups（P 〈0.01）; at 1 month, 3 months and 6 months after operation, excellent and good rate difference was not statistically significant between the 2 groups （ P 〉 0.05 ）. There were significant differences on VAS score and ODI score of two group after treatment compared with before group [ treatment group： （ 3.0 ± 1.9 ） scores vs （ 6.4 ± 1.2 ） scores, （ 17.5 ± 7.5 ） scores vs （47.1 ± 8.3）scores; control group： （4.9 ± 1.7） scores vs （6.4 ± 2.8） scores, （23.6 ± 10. 1 ） scores vs （47.0 ± 7.9） scores] （P 〈 0.01 ）, but the improvement range in treatment group was significantly better than that in control group （P 〈0.01 ）. The incidences of complication in treatment group （18.4% ,9/49 ）was significantly lower than that in control group （61.2% ,30/49） （P 〈 0.01 ）. Conclusion Fufang nanxing zhitong Plaster cooperated with PLDD is a valid method of treating discogenic back pain.
