摘要
目的:探讨支气管镜肺泡灌洗联合序贯机械通气治疗COPD合并呼吸衰竭的临床疗效。方法:选取2011年5月-2013年4月本院收治的58例COPD合并呼吸衰竭患者,按照随机数字表法将其分为对照组和治疗组各29例。对照组给予常规的对症治疗和有创-无创序贯机械通气治疗,治疗组在对照组的基础上行经纤维支气管镜肺泡灌洗。检测两组患者治疗前后的动脉血气分析指标PaO2、PaCO2及pH,同时记录两组患者的机械通气时间、住院时间及并发症的发生率。结果:治疗后,两组患者的PaO2、PaCO2及pH值较治疗前均有明显改善;且治疗组明显优于对照组,差异均有统计学意义(P<0.05)。治疗组患者的机械通气时间(12.5±4.5)d明显少于对照组的(21.5±6.6)d,住院时间(19.7±5.6)d明显少于对照组的(30.8±6.6)d,并发症发生率3.4%明显低于对照组的37.9%,比较差异均有统计学意义(P<0.05)。结论:支气管镜肺泡灌洗联合序贯机械通气治疗COPD合并呼吸衰竭具有疗效确切、安全可靠等优点,可明显缩短机械通气时间,降低并发症的发生率。
Objective:To observe the clinical effects of bronchia alveolar wash combined with mechanical ventilation on COPD patients complicated with respiratory failure.Method:Fifty-eight COPD patients complicated with respiratory failure were selected in our hospital from May 2011 to April 2013,they were randomly divided into the control group and the observation group,29 cases in each group.The control group was given conventional symptomatic treatment and invasive/non-invasive sequential mechanical ventilation,and the observation group was given bronchia alveolar lavage on the basis of the control group.PaO2,PaCO2 and pH of two groups were detected before and after treatment,and mechanical ventilation time,length of stay and incidence of complication were recorded.Result:After treatment,PaO2,PaCO2 and pH of two groups both improved and the observation group were better than the control group,the differences had statistically significant(P〈0.05).The duration of mechanical ventilation in the treatment group was (12.5±4.5)d,it was significantly less than that in the control group(21.5±6.6)d,hospitalization time(19.7±5.6)d was significantly less than that in the control group(30.8±6.6)d,the incidence of complications was 3.4%,it was significantly lower than the control group of 37.9%,the differences had statistically significant(P〈0.05).Conclusion:Bronchia alveolar lavage combined with mechanical ventilation has curative effect,is safe and reliable,and can significantly shorten mechanical ventilation time and reduce the incidence of complication.
