摘要
目的:研究分析硬膜外自控镇痛在老年骨科手术患者中的镇痛效果及对其术后应激的影响。方法:选取于本院进行手术治疗的90例老年骨科患者为研究对象,将90例患者随机分为对照组(静脉自控镇痛组)45例和观察组(硬膜外自控镇痛组)45例,然后将两组患者术后不同时间的疼痛程度、血清疼痛依存指标与围术期的血清氧化应激、应激激素水平进行比较。结果:观察组术后6、12h与48h的疼痛程度、血清疼痛依存指标均低于对照组,术后1、3d及5d血清氧化应激、应激激素水平也好于对照组,差异均具有统计学意义(P<0.05)。结论:硬膜外自控镇痛在老年骨科手术患者中的镇痛效果较好,对患者术后应激的控制效果也较好。
Objective, To study and analyze the analgesic effects of patient-controlled epidural anal- gesia in elderly patients underwent orthopedics operation and the influence for their postoperative stress. Methods：A total of 90 elderly patients underwent orthopedics operation in our hospital from July 2010 to August 2013 were selected and randomly divided into control group（patient-controlled intravenous analge- sia group, 45 cases） and observation group（patient-controlled epidural analgesia group,45 cases）,then the pain degree, serum pain dependence indexes at different times after the operation and serum oxidative stress, stress hormones during the perioperative period of two groups were compared. Results- The pain de- gree,serum pain dependence indexes at 6 h,12 h and 48 h after the operation were all lower than those of control group,serum oxidative stress,stress hormones at lst,3rd and 5th day after the operation were bet- t-r than those of control group （P all0.05）. Conclusion= The analgesic effect of patient-controlled epi- dural analgesia in the elderly patients underwent orthopedics operation is good, it also shows good effects on postoperative stress.
