摘要
目的讨论CT诊断周围型肺癌时的图像特征及临床意义。方法回顾性分析2010年5月至2012年5月间,经术后病理证实的68例周围性肺癌患者的资料,总结CT诊断时典型的图像特征。结果右肺49例,占72.1%;左肺19例,占27.9%。病灶平均面积(6.9±0.4)cm×(3.2±0.1)cm;病灶区呈不规则斑片状阴影24例(35.3%),阴影呈圆形、类圆形44例(64.7%);实性高密度影28例(41.2%),低密度影28例(41.2%),密度不均匀13例(19.1%);磨砂玻璃样结节27例(39.7%),空泡征样结节10例(14.7%);蜂房样结节11例(16.2%),软组织密度结节20例(29.4%);边缘光滑整齐7例(10.3%),边缘不光滑平整清晰61例,占89.7%;凹陷征39例(57.4%)、血管集束征29例(42.6%)。结论熟练掌握周围型肺癌的影像学特点,通过对肺部CT诊断时的图像资料分析,做到早期准确诊断,为确诊和救治争取时机,对延长患者生存时间有重要意义。
Objective To explore CT image characteristics of the peripheral lung cancer and its clinical effect. Methods The date of 68 patients with lung cancer confirmed by postoperative pathology from May 2010 to May 2012 was analysis retrospectively,and the CT diagnosis of typical image characteristics was summarized. Results The fight lung,49 cases, accounted for about 72. 1%, the left lung, 19 cases, accounted for about 27. 9%. Average lesion area was （ 6. 9 ＋ 0.4） cm × （ 3.2 ＋ 0. 1 ） cm. Lesion area irregular patchy shadows had 24 cases （ 35.3% ） , assumes the circular shadow, classes, circular 44 cases （64. 7% ） ;Solid high density shadow 28 cases （41.2%） ,Solid low density shadow 28 cases （41.2%） ,low density shadow uneven density 13 cases （19. 1% ） ;Frosted glass sample nodules 27 cases （39. 7% ）, cavity sign sample nodules 10 cases （14. 7% ） ;Honeycomb sample nodules 11 cases （16. 2% ） ,soft tissue density nodules 20 cases （29. 4% ） ;Smooth edge had 7 cases （10. 3% ） ,the edge which was not smooth level and clear had 61 cases ,aeeountingfor 89. 7% ;Suleus sign had 39cases （57.4%） ,blood vessels and cluster sign had 29 cases （42.6%）. Conclusions To master imaging characteristics of peripheral lung cancer,through analysising the CT image data of lung, early accurate diagnosis has important significance for diagnosis, treatment and survival period.
