摘要
目的对腰椎间盘突出症椎间盘摘除后患者Quadrant微创系统操作下,安装Coflex系统可实现脊柱外科手术的微创性及非融合性。文中探讨采用该技术治疗腰椎间盘突出症手术的方法及临床疗效。方法 2011年9月至2012年3月,采用Quadrant微创系统操作下Coflex非融合固定治疗腰椎间盘突出症患者18例,均为单节段椎间盘突出症,L3/L4椎间盘突出症10例,L4/L5椎间盘突出症8例。评估患者术前、术后3个月、术后6个月的腰腿痛VAS、Oswestry功能障碍指数(oswestry disability index,ODI)、腰椎活动度(range of lumbar motion,ROM)等指标。结果所有患者均顺利完成手术,术后随访时间均大于半年,术后3个月、6个月随访时与术前相比,VAS、ODI指标均显著改善(P<0.05),并且保持了良好的腰椎活动度(P<0.05)。结论在严格掌握手术指征的情况下,Quadrant微创系统可以在微创理念的指导下进行腰椎间盘突出症的相关手术治疗,能有效缓解疼痛、改善功能同时保护责任节段的活动幅度,并可以安全有效地放置脊柱非融合固定系统,患者获得较好的手术疗效。
Objective The use of Coflex via the Quadrant minimally invasive system can help realize minimal invasiveness and non-fusion in spine surgery. Our study was to evaluate the effect of Coflex via the Quadrant minimally invasive system in the treat- ment of lumbar disc herniation. Methods Eighteen patients with lumbar disc herniation, 14 males and 4 females aged 34 - 61 ( mean 41.5 ) years were treated by Coflex via the Quadrant minimally invasive system. The course of disease was 6 - 35 ( mean 12.4 ) months. All the patients were diagnosed with single segmental disc herniation: 8 with L.3/4 and 7 with L4/5. The Visual Analogue Scale ( VAS), Oswestry disability index (ODI) and range of lumbar motion (ROM) were recorded before and at 3 and 6 months after surgery. Results Operations were successfully completed in all the patients, and all were followed up for more than 6 months. Com- pared with preoperation, VAS and ODI were significantly improved at 3 and 6 months after surgery ( P 〈 0.05 ), and good lumbar activ- ity was maintained ( P 〈 0.05 ). Conclusion On condition of strict indication, Coflex via the Quadrant minimally invasive system can effectively alleviate pain, protect spinal activity, and produce desirable results in the treatment of lumbar disc herniation.
