骨折患者术后医院感染的相关因素分析 被引量:3

Related factors of postoperative nosocomial infections in fracture patients

摘要 目的分析骨折患者术后医院感染的麻醉相关因素,对麻醉因素所致的感染率进行总结并提出有效的防范措施,为临床感染控制提供必要的指导和依据。方法回顾性分析1823例骨折手术患者的临床资料,统计感染例数、感染率、感染部位分布、病原菌分布以及与感染相关的手术及麻醉因素。结果调查1823例,感染56例,感染率为3.07%;感染部位以下呼吸道、上呼吸道和手术切口为主,分别占33.93%、21.43%、19.64%;检出病原菌中革兰阴性菌42株占75.00%、革兰阳性菌14株占25.00%;分布最多的3种病原菌为铜绿假单胞菌、大肠埃希菌和金黄色葡萄球菌,分别占30.36%、19.64%、17.86%;经χ2检验,医院感染与骨折类型、切口类型、麻醉方式、手术时间、是否使用一次性过滤器和是否使用止吐药物有关(P<0.05)。结论骨折患者医院感染率相对较高,应根据患者骨折类型和麻醉特点,尽可能采取相应措施,降低医院感染率的发生。 OBJECTIVE To analyze the related factors of anesthesia-associated nosocomiai infections in fracture patients, observe the incidence of the infections, and put forward the effective prevention measures so as to provide guidance tor the prevention and control of the clinical infections. METHODS The clinical data of 1828 cases of patients who underwent the fracture surgery were analyzed retrospectively, and the cases of infections, infection rate, distribution of infection sites, distribution of pathogens, and operation or anesthesia-associated infections were statistically analyzed. RESULTS Of totally 1823 cases of patients investigated, the infections occurred in 56 cases with the incidence rate of 3.07%, the lower respiratory tract, upper respiratory tract, and surgical incision were the main infection sites, accounting for 33.93%, 21.43%, and 19.64% ,respectively. Among the detected pathogens, there w, re 42 (75.00%) strains of gram-negative bacteria and 14 (25.00%) strains of gram-positive bacterla%Pseudomonas aeruginosa (30. 36%), Escherichla colf (19.64%) and Staphylococcus aureus (17.86%) were the predominant species of pathogens, The result of chi-square test revealed that the incidence of nosocomial infections was related to the classification of fracture, types of incision, anesthesia mode, operation duration, use of disposable filters, or use of antiemetics (P〈0.05). CONCLUSION The incidence of nosocomial infections in the fracture patients is relatively high, the corresponding measures should be adopted as much as possible according to the classification of fraciure and the anesthesia approaches so as to reduce the incidence of nosocomial infections.
