摘要
目的探讨NT-proBNP用于心功能不全诊断的临床意义。方法回顾性分析我院2011年1月-2012年12月收治心功能不全患者290例心功能分级,左心室射血分数(LVEF)及血浆NT-proBNP水平等临床资料。结果心功能分级Ⅰ级、Ⅱ级、Ⅲ1级及Ⅳ级者NT-proBNP水平分别为(134.5±27.0)pg/ml,(495.2±132.4)pg/ml,(1344.8±315.9)pg/ml,(1602.7±398.3)pg/ml;LVEF水平分别为(68.1±12.3)%,(60.8±11.4)%,(54.5±13.3)%,(46.2±9.7)%;其中血浆NT-proBNP水平<100pg/ml者约占总人数12.8%,而LVEF水平>50%者约占总数65.5%。结论NT-proBNP用于心功能不全诊断有助于降低临床漏诊率;其水平与心功能分级呈正相关,与LVEF水平呈负相关。
Objective To investigate the clinical value of N-terminal prohormone of brain natriuretic peptide (NT- proBNP) in the diagnosis and treatment of ventricular dysfunction. Methods A retrospective analysis was performed on the clinical data of 290 patients with ventricular dysfunction who were admitted to our hospital from January 2011 to December 2012, including plasma NT-proBNP level, cardiac functional grades, and left ventricular ejection fraction (LVEF) Results The NT- proBNP levels of patients whose cardiac functional grades were Ⅰ,Ⅱ, Ⅲ, and Ⅳwere 134.5±27.0 pg/ml, 495.2±132.4 pg/ ml, 1344.8±315.9 pg/ml, and 1602.7±398.3 pg/ml, respectively; LVEF levels were 68.1±12.3%, 60.8±11.4%, 54.5± 13.3%, and 46.2±9.7%, respectively. The patients whose plasma NT-proBNP levels were lower than 100 pg/ml and whose LVEF levels were higher than 50% accounted for 12.8% and 65.5%, respectively. Conclusion The application of NT-proBNP in diagnosis and treatment of ventricular dysfunction can help to reduce the clinical misdiagnosis rate; NT- proBNP level is positively correlated with cardiac functional grade but negatively correlated with LVEF level.
