摘要
目的评价短型股骨粗隆部锁定接骨板与空心钉内固定治疗股骨颈骨折的临床疗效及特点。方法101例股骨颈骨折分别采用短型股骨粗隆部锁定接骨板(钢板组,53例)与空心钉(空心钉组,48例)内固定治疗,并进行对照研究。结果所有患者均获随访18.24个月,平均19。4个月。两组手术时间、术中出血量、住院时间差异无统计学意义(P〉0.05);两组术后12、18个月髋关节功能Harris评分差异无统计学意义(P〉0.05),但术后24个月钢板组明显优于空心钉组(P〈0.05);术后钢板组并发症发生率为11.3%,空心钉组并发症发生率为25.0%,钢板组明显低于空心钉组(P=0.027)。结论采用闭合复位小切口、短型股骨粗隆部锁定接骨板微创内固定治疗股骨颈骨折承接了加压空心钉创伤小、循环破坏少、有效动力加压作用同时.因其静力固定的稳态作用可减少内固定松动、失败及股骨颈短缩等并发症的发生,功能恢复好,尤其对不具备人工髋关节置换生理条件的老年患者有重要意义。
Objective To investigate the clinical efficacy and characteristics about therapy of femoral neck fracture using short femoral proximal locking plate and cannulated screw. Methods A total of 101 cases of femoral neck fractures were treated randomly by using short femoral proximal locking plate or cannulated screw, and analysis of surgical methods and clin- ical effects were performed. Results All patients were followed up for 18 to 24 months (mean 19.4 months). There were no significant difference in operation time, blood loss, time of hospital stays, and postoperative 12, 18 months hip Harris scores between plate group and cannulated screw group (P 〉0.05). However, there was significant difference between postoperative 24 months hip Harris scores(P 〈0.05), plate group was better than screw group. The incidence of complications was 11.3% in plate group,and 25.0% in eannulated screw group,there was significant difference between two groups (P =0.027). Conclusion Therapy of femoral neck fracture using short femoral proximal locking plate with less cut and closed fracture restoration has not only the merits of cannulated screw fixation such as less operating trauma, less vascular lesion and fracture surface compres- sion, but also has static firm station which can avoid femoral neck shortening, and failure of fixation. The therapy has a good outcome and less complications, and is essential for old patients with femoral neck fracture but have not probable conditions to hip replacement surgery.
