摘要
目的 研究采用颊脂垫瓣修复术治疗口腔恶性肿瘤切除后组织缺损的临床效果,探讨颊脂垫转移瓣修复口腔恶性肿瘤切除术的手术技巧、术后转归及对颊、腭部缺损的适应证的疗效.方法 选择2010年1月～2012年12月在广元市第一人民医院进行治疗的30例颊脂垫瓣修复口腔恶性肿瘤切除后组织缺损的患者资料,缺损范围包括后颊和腭部,其中缺损最小面积为2.0 cm×2.5 cm,最大面积为5.0 cm×4.5 cm.研究分析患者的相关资料,比较采用颊脂垫转移瓣进行修复的过程和效果.结果 全部患者治疗效果良好.术后7d颊脂垫瓣出现明显的水肿,表面覆盖有伪膜,伪膜在14 d内完全消失.2～3周后,颊脂垫瓣水肿逐渐消退,其表面逐渐上皮化,2个月后再生的黏膜呈粉红色,质地光滑,类似正常口腔黏膜.结论 颊脂垫瓣由于制取较为方便,操作相对安全,切口隐蔽,术后并发症较少,因而成为了修复口腔恶性肿瘤的首要方式,应在临床中推广使用.
Objective To study the clinical effect of excision of the tissue repair of buceal fat pad flap for the treat- ment of malignant tumors in the oral defect, and to explore the buccal fat pad flap repair of oral cancer metastasis re- section surgical technique, postoperative outcome and right hip defect indications of efficacy. Methods 30 cases of pa- tients who used the excision of the tissue repair of buccal fat pad flap for the treatment of malignant tumors in the oral defect were selected in the Guangyuan First People＇s Hospital, during January 2010 to December 2012 were selected. The area of defects included after buccal and palatal, of which a minimum area of defect was 2.0 cm x 2.5 cm, the maximum area of 5.0 cm x 4.5 cm. The related data of the patients were analyzed to compare the process and the ef- fect of the excision of the tissue repair of buccal fat pad flap for the treatment of malignant tumors in the oral defect. Results All the patients were treated with good effect. After 7 d, the buccal fat pad flap appeared obvious edema, cov- ered with pseudo membrane and the pseudo membrane disappeared completely in 14 d. After 2-3 weeks, the buccal fat pad flap edema gradually subsided. 2 months later, the regeneration of the mueosa was pink, smooth texture and it was similar to normal oral mucosa. Conclusion Buccal fat pad flap preparation is more convenient because the opera- tion is relatively safe, hidden incision, less postoperative complications, and thus become the primary way of repairing oral malignancy should promote the use of clinical.
