超声测量心外膜脂肪组织厚度与冠心病的相关性研究 被引量:10

Study on association between epicardial adipose thickness measured by echocardiography and coronary heart disease

摘要 目的评价心外膜脂肪组织厚度(EAT)与冠心病(CHD)的相关性及其诊断预测价值。方法共298例符合入选标准的对象行冠状动脉造影,符合CHD诊断标准的200例为CHD组,余98例为对照组,通过经胸彩色多普勒超声心动图测量EAT,用SPSS 17.0软件进行数据分析,比较两组EAT差异。结果 CHD组EAT大于对照组[(5.44±1.50)mm比(4.52±1.31)mm,P<0.01];EAT与CHD呈正相关(OR=1.598);EAT诊断CHD的受试者工作特征曲线下面积:男性为0.67,诊断敏感度59%,特异度82%,诊断切点值为5.06 mm;女性为0.71,诊断敏感度58%,特异度72%,诊断切点值为4.88 mm。结论 EAT是CHD的一个独立危险因素,经胸彩色多普勒超声心动图测量EAT可以作为CHD预测评估的一个有效指标。 Objective To study the association between epicardial adipose thickness (EAT) and coronary heart disease (CHD) and the diagnostic and predictive value of EAT for CHD. Methods EAT were measured by transthoracic echocardiography for all the 298 cases enrolled in this study. According to the diagnostic standard of CHD, all subjects were divided into two groups: CHD and control group. All data were analyzed by SPSS17.0 software. Results EAT was significantly greater in CHD group than that in control group [ (5.44 ± 1.50) mm vs. (4. 52 ± 1.31 ) ram, P 〈0. 01 ]. EAT was positively correlated with CHD ( OR = 1. 598 ). The area under ROC curve of EAT measured by transthoracic echocardiography in the diagnosis of CHD was 0. 67 in men and 0. 71 in women, the diagnostic sensitivity was 59% in men and 58% in women, the diagnostic specificity for men was 82% and 72% for women, appropriate cut-off was 5.06 mm for men and 4. 88 mm for women. Conclusions EAT is an independent risk factor of CHD. EAT measured by transthoracic echocardiography may serve as an useful predictive marker of CHD.
