摘要
目的观察机械通气联合纤维支气管镜肺泡灌洗术治疗呼吸衰竭脱机困难患者的疗效与安全性。方法选取58例ICU机械通气患者因肺部感染脱机困难大于5d的患者随机分成两组,对照组为机械通气+普通吸痰、抗感染、解痉、营养支持等治疗;治疗组在原有治疗基础上接受纤维支气管镜肺泡灌洗术。结果治疗组血气指标改善、总有效率明显提高。结论机械通气联合纤支镜肺泡灌洗术,治疗呼吸衰竭脱机困难患者,疗效确切、操作方便,值得在ICU推广应用。
Objective To observe the efficacy and safety of mechanical ventilation combined with alveolar wash by bronchoscope on respiratory failure patients who were difficult to be off-line.Methods 58 cases of ICU patients received mechanical ventilation for more than 5 days who were difficult to be off-line for luny infection,were randomly divided into two groups,The control group were treated with mechanical ventilation＋suctioning,anti-inflammatory,antispasmodic,nutrition support.bronchoscope lavage was appled in the treatment group,accepted on the basis original treatment.Results The blood gas analysis and the total efficiency hasd,improved significantly in the treatment.Conclusion Mechanical ventilation combined with bronchoscope lavage for breathing the failure offline difficult patients,might be efficacy,easy to operate,worth to applicate in ICU.
