摘要
目的探讨微创经皮钢板接骨(MIPPO)技术结合锁定钢板治疗胫骨远端骨折的临床效果。方法选取本院2009年10月~2012年11月收治的胫骨远端骨折患者46例,根据手术方法分为两组,23例患者实施切开复位解剖型钢板内固定治疗为对照组,23例患者实施MIPPO技术结合锁定钢板内固定治疗为观察组.术后随访6个月,比较两组患者的临床指征、临床疗效、并发症情况。结果观察组手术时间(46.1±12.4)min、术中出血量(105.7±26.8)mL、伤口愈合时问(7.2±1.3)d、骨痂生成时间(5.1±0.6)周、骨折愈合时间(6.8±1.1)周,均明显小于对照组(82.0±15.3)min、(186.5±31.9)mL、(9.1±1.7)d、(7.4±1.0)周、(8.9±1.4)周,观察组手术治疗优良率(95.7%)明显高于对照组(73.9%),差异有统计学意义(P〈0.05)。观察组术后并发症发生率(8.7%)虽低于对照组(21.7%),但差异无统计学意义(P〉0.05)。结论MIPPO技术结合锁定钢板内固定是治疗胫骨远端骨折的有效方法.可显著改善患者的临床指征,提高手术治疗优良率,且并发症较少,值得临床推广使用。
Objective To investigate clinical effect of MIPPO technology combined with locking plate in treatment for distal tibia fractures Methods The 46 patients with distal tibia fractures were selected in our hospital from October 2009 to November 2012.According to operation methods, patients were randomly divided into two groups. 23 patients treated anatomical plate internal fixation as control group. 23 patients treated MIPPO technology combined with locking plate internal fixation as observation group. Patients were followed up for 6 months. Clinical indications, clinical effect, complications were compared between two groups. Results Operation time (46.1± 12.4)min, intraoperative blood loss (105.7±26.8) mL, wound healing time (7.2±1.3)d, callus formation time (5.1±0.6)weeks, fracture healing time (6.8±1.1)weeks in observation group were significantly lower than control group (82±15.3)min, (186.5±31.9)mL, (9.1± 1.7)d, (7.4±l.0)weeks, (8.9±l.4)weeks. Operation cure rate (95.7%) in observation group was significantly higher than the control group (73.9%). The difference was statistically significant(P 〈 0.05). Incidence of postoperative complications in observation group (8.7%) was lower than control group(21.7%), the difference was not statistically significant (P 〉 0.05). Conclusion MIPPO technology combined with locking plate internal fixation is an effective method in treatment of distal tibial fractures,which can significantly improve clinical indications and operation cure rate.That has less complications and worth clinical use.
