摘要
目的探讨超声心动图对胎儿先天性心脏病产前筛查与诊断临床价值。方法选取本院2010年2月～2013年2月行常规超声检查的孕妇2568例,通过超声心动图进行产前筛查,观察胎儿四腔心切面、左右室流出道切面、上下腔静脉长轴切面、三血管气管断面观察、主动脉弓及动脉道弓切面,并对检查结果进行追踪和随访,以便进行验证。结果产前超声心动图诊断为先天性心脏病的14例患者均确诊,完全性心内膜垫缺损3例、室间隔缺损5例、房间隔缺损1例、复杂先天性心脏病部分合并心外畸形2例、单心室1例、左右心发育不良2例。漏诊病例为室间隔缺损1例。结论应用超声心动图对胎儿先天性心脏病的产前筛查是准确可行的,对围生期临床正确处理有重要的指导作用,值得临床应用和推广。
Objective To investigate the clinical value of echocardiography in the prenatal screening and diagnosis of congenital heart disease.Methods 2568 pregnant women who had received routine ultrasound examination in our hos- pital from February 2010 to February 2013 were selected.Prenatal screening was conducted by echocardiography.The fetal four-chamber heart section,left and right ventricular outflow tract section,superior and inferior vena eava long axis section,three-vessel trachea cross-section,and aortic arch and arterial passage arch section were observed.The test re- suits were tracked and followed up for the purpose of verification.Results 14 patients diagnosed with congenital heart diseases by prenatal eehocardiography were confirmed,3 patients had complete endocardial cushion defect,5 patients had ventricular septal defect,1 patient had atrial septal defect,2 patients had complex congenital heart disease partially complicated with heart malformation,1 patient had single ventricle and 2 patients had right and left heart hypoplasia. One missed diagnosis was ventricular septal defect.Conclusion The prenatal screening of fetal congenital heart disease by echocardiography is accurate and feasible and plays an important role in guiding the correct perinatal clinical treat- ment,thereby worthy of clinical application and promotion.
