摘要
目的探讨多种药物混合持续湿敷治疗化疗药渗漏所致皮肤损害的效果。方法将2009年1月至2010年12月收治行外周浅静脉途径化疗而发生渗漏的肿瘤患者32例作为对照组，应用利多卡因100mg＋地塞米松3mg＋0．9％氯化钠注射液5ml在穿刺点周围环形多点皮下注射；选择2011年1月至2012年6月行外周浅静脉途径化疗而发生渗漏的肿瘤患者33例作为治疗组，治疗组在穿刺点周围环形多点皮下注射的基础上，实施多种药物混合（50％硫酸镁、2％利多卡因、地塞米松、5％碳酸氢钠、山莨菪碱、维生素B。0．9％氯化钠注射液）配制溶液浸透的纱布敷料局部持续湿敷，治疗结束后观察治疗效果。结果治疗组临床治疗总有效率为100．0％，显效率为75．8％；对照组临床治疗总有效率为78．1％，显效率为46．9％，两组治疗后总有效率和显效率比较差异有统计学意义（Х^2值分别为5．97，5．73；P〈0．05）。治疗组24h内肿胀消失者16例，对照组7例，而在73—120h内肿胀消失者治疗组4例，对照组12例，差异有统计学意义（Х^2值分别为5．03，5．64；P〈0．05）。结论在穿刺点周围环形多点皮下注射的基础上，实施多种药物混合配制溶液浸透的纱布敷料局部持续湿敷能有效治疗化疗药物渗漏对皮肤及皮下组织所造成的损害。
Objective To explore the effects of continuous compress with multiple drugs mixed in the treatment of skin lesions caused by chemotherapeutic drugs extravasation. Methods Totals of 32 cases of cancer applied peripheral superficial vein chemotherapy and occurred drugs extravasation from Jan. 2009 to Dec. 2010 as control group, lidocaine 100 mg, dexamethasone 3 mg and saline 5 ml were used through circular hypodermic injection around the point of puncture; Totals of 33 cases applied peripheral superficial vein chemotherapy and occurred drugs extravasation from Jan. 2011 to Jun. 2012 as treatment group, in addition to the therapeutic methods mentioned above, local continuous compress with gauze soaked by multiple drugs （50% magnesium sulfate, 2% lidocaine, dexamethasone, 5% sodium bicarbonate, anisodamine, vitamin B,=, saline） mixed were implemented. Results The treatment group had a higher overall response rate （ 100.0% ）, and more effective rate（75.8% ）, with the control group overall response rate （78.1% ）, and more effective rate （46.9%）, and the difference was statistically significant （ Х^2 = 5.97,5.73, respectively; P 〈 0. 05 ）. The cases of swelling disappeared within 24 hours of treatment group was significantly more than that of control group（ 16 vs 7 ）, while the cases of swelling disappeared within 73 - 120 hours of treatment group was significantly less than that of control group（4 vs 12）, with significant difference（ Х^2 = 5.03,5.64;P 〈 0. 05 ）. Conclusions On the base of using lidocaine to cure skin lesions caused by chemotherapeutic drugs extravasation, continuous compress with gauze soaked by multiple drugs mixed is an effective therapy.
