二维斑点追踪技术与组织速度成像技术评价慢性心力衰竭患者心肌收缩后缩短对比研究 被引量:9

Comparison of 2D-speckle tracking imaging and tissue velocity imaging in evaluating post-systolic shortening in patients with chronic heart failure

摘要 目的探讨二维斑点追踪技术(2D-speckle tracking imaging,2D-STI)与组织速度成像技术(tissue velocityimaging,TVI)检测慢性心力衰竭(chronic heart failure,CHF)患者局部心肌在等容舒张期出现收缩后缩短(post-systolic shortening,PSS)的一致性。方法 CHF患者30例(CHF组)与体检健康者30例(对照组),采用2D-STI与TVI检测2组左心室节段心肌PSS,比较2D-STI检出PSS与TVI的一致性。结果 CHF组采用2D-STI检测CHF峰值应变低于对照组(P<0.05),峰值应变达峰时间较对照组延长(P<0.05);采用TVI检测CHF峰值速度低于对照组(P<0.05),峰值速度达峰时间较对照组延长(P<0.05);CHF组2种技术检测PSS的阳性率均高于对照组(P<0.05);CHF组2D-STI对PSS的检出率为29.6%,TVI对PSS的检出率为26.1%,二者比较差异无统计学意义(P>0.05),具有良好一致性(Kappa值=0.83,P<0.05)。结论采用2D-STI检测CHF患者心肌局部PSS与TVI有较高一致性,可作为检测CHF患者局部PSS的新方法。 Objective To compare 2D-speckle tracking imaging(2D-STI) and tissue velocity imaging(TVI) in evaluating post-systolic shortening in patients with chronic heart failure(CHF),and to discuss the feasibility of 2D-STI in the assessment of post-systolic shortening(PSS) in isovolumic relaxation phase.Methods Thirty CHF patients(CHF group) and 30 healthy volunteers(control group) were assessed with STI and TDI,and compared the consistency of the results of two techniques.Results Peak values of CHF by both 2D-STI and TVI were lower,the time to peak was longer obviously in CHF group than that in control group(P0.05).The detection rates of PSS with 2D-STI and with TVI were 29.6% and 26.1% in CHF group,which showed no significant difference(P0.05).These two techniques had a high consistency(Kappa=0.83,P0.05).Conclusion To detect PSS in CHF with STI and TVI has a higher consistency.2D-STI can be used as a new technique to detect PSS in patients with CHF.
