新型重组禽流感病毒(H7N9)性肺炎的影像学初步观察 被引量:38

摘要 目的探讨新型重组禽流感病毒(H7N9禽流感病毒)性肺炎的影像学表现。方法收集上海市公共卫生临床中心2013年4月313至11日收治的8例确诊为人感染H7N9禽流感病毒患者的临床资料(男6例、女2例),对其影像表现进行分析。结果8例患者均行X线和CT检查,7例累及3个肺叶以上;病变发生于左肺下叶和右肺下叶最多,各7例次;6例一侧肺病变分布明显多于对侧;8例均见磨玻璃密度影(GGO)、实变和充气支气管征,5例小叶间隔增厚,7例出现少量胸腔积液。结论H7N9禽流感肺炎具有一定的影像学特征,一般表现为单侧肺感染为主、多肺叶广泛受累的GGO和实变、充气支气管征常见,病情严重时常伴有少量胸腔积液;X线和CT检查对诊断和判断疗效有重要作用。 Objectives To explore the imaging findings of human infection of novel avian-origin Influenza A (H7N9) virus. Methods Data were collected from 8 patients with novel avian-origin Influenza A (H7N9) virus infection in Shanghai public health clinical center since April 3,2013 to April 11, 2013. Clinical and imaging data of the 8 patients were retrospectively analyzed. Eight patients included 6 males and 2 females. Results All of the 8 patients underwent X-ray and spiral CT examinations. The lung lesions involving over 3 lobes of the whole lung were seen in 7 cases. Lesions were detected in left lower lobe and right lower lobe in 7 cases respectively. And the distribution of lesions mainly located on one side of the lung in 6 cases. All the 8 cases showed extensive GGO, consolidation and air bronchogram on the CT images. The interlobular septal thickening was visible in 5 cases. Pleural effusion was seen in 7 cases. Conclusions Novel avian-origin Influenza A (H7N9) pneumonia shows some imaging characteristics. The disease generally involves unilateral pulmonary and multiple lobes. The imaging findings include extensive GGO and consolidation, usually with air bronchograms. In severe cases, small amount of pleural effusion can be seen. X-ray and spiral CT examinations play an important role in the diagnosis and therapeutic evaluation of the disease.
