摘要
目的:观察妊娠期糖尿病(GDM)患者不良心理情绪特征,探讨有效心理护理干预措施。方法:将93例GDM患者随机分为对照组40例与观察组53例,对照组实施常规治疗与护理,观察组在对照组基础上加强心理护理干预措施;比较两组患者不良心理情绪、GDM知识知晓情况、血糖控制情况、治疗信心、依从性、焦虑自评量表(SAS)和抑郁自评量表(SDS)评分、对医护工作的满意度及产后转内科治疗的情况。结果:观察组GDM知识知晓率、治疗信心、血糖控制良好率、患者依从性及满意度均明显高于对照组(P<0.01),不良心理情绪发生率、SAS及SDS评分、转内科治疗率明显低于对照组(P<0.01)。结论:GDM患者存在各种不良心理情绪,有效的个性化心理护理能够显著改善患者不良心理反应,提高GDM知识知晓率、治疗信心、治疗依从性、血糖控制良好率和对患者满意度,同时降低产后转内科治疗率。
Objective:To observe the adverse psychological characteristics of patients with gestational diabetes mellitus (GDM) in order to find effective psychological nursing intervention measures. Methods :93 patients with GDM were randomly divided into the control group ( n = 40 ) and the observation group ( n = 53 ). The conventional treatment and nursing care was implemented in the control group and the enhanced psychological nursing intervention was additionally given to the patients in the observation group. The adverse psychology and grasp of GDM knowledge of the patients, control of blood sugar, confidence in the treatment, compliance, the scores of Self - rating Anxiety Scale (SAS) and Self - rating Depression Scale ( SDS ), satisfaction and the treatment outcome after transferring to department of intemal medicine were compared between the two groups. Results : The awareness rate of GDM knowledge, confidence in the treatment, good rate of blood glucose control, compliance and satisfaction of the patients were significantly higher and better in the observation group than the control group ( P 〈 0.01 ) ;the incidence of adverse psychology, scores of SAS and SDS and the treatment rate of transferring to department of internal medicine were significantly lower in the observation group than the control group ( P 〈 0.01 ). Conclusion : The patients with GDM have a variety of adverse psychology and the effective personalized psychological nursing care can significantly alleviate the reaction to adverse psychology, improve the awareness rate of GDM knowledge, confidence in the treatment, treatment compliance, good rate of blood glucose control and patient satisfaction and can reduce the rate of transfer as well.
