摘要
目的:研究蛭黄洗液的制备,观察经腹腔镜置管行蛭黄洗液腹腔保留灌洗治疗非胆源性重症胰腺炎的疗效。方法:以水蛭、大黄等中药为原料,制成蛭黄洗液,将非胆源性重症胰腺炎63例随机分为蛭黄洗液灌洗组、盐水灌洗组和保守治疗组,每组21例。24 h内给予禁食、抗生素、抑酶等常规治疗,48～72 h内按组分别给予蛭黄洗液保留灌洗、盐水灌洗1周和保守治疗。结果:蛭黄洗液灌洗组治愈15例、盐水灌洗组治愈9例和保守治疗组治愈4例。各组均有1例死亡。结论:治疗非胆源性重症胰腺炎,经腹腔镜置管蛭黄洗液腹腔保留灌洗,能够改善症状、缩短病程及降低部分术后并发症发生率。
Objective：To study the preparation of Zhihuang lotion and observe the therapeutic efficacy of non-biliary severe pancreatitis treated with Zhihuang lotion intraperitoneal retained lavage through laparoscopic catheter.Method：Zhihuang lotion consists of leeches,rhubarb and other traditional Chinese herbs.63 patients were involved in the research,who were Randomly separated into the Zhihuang lotion lavage group（n=21）,the saline lavage group（n=21）and the conservative treatment group（n=21）.All of the patients were given conventional treatment within 24 h,such as fasting,antibiotics,enzymes suppression,and then,within 48-72hours,three groups were separately offered Zhihuang lotion lavage,saline lavage and conservative treatment for 1 weeks.Result：15patients were cured in the Zhihuang lotion lavage group,while 9cases in the saline lavage group and 4cases in the conservative treatment group.One patient died in each group.Conclusion： Non-biliary severe pancreatitis treated with Zhihuang lotion intraperitoneal retained lavage through laparoscopic catheter could improve symptoms and shorten the course and reduce the incidence of some postoperative complications.
