CT and MRI manifestations of solitary fibrous tumor

摘要 目的:探讨CT和MRI在孤立性纤维性肿瘤(SFT)中的诊断价值。方法:回顾性分析21例经手术或病理证实的SFT的CT和MRI表现,21例均行CT扫描,其中8例行MRI扫描。分析肿瘤的位置、大小、形态、密度、信号及强化方式。结果:SFT最大径线3.0~22.6cm(平均9.8cm)。CT表现为边界清楚或不清楚的软组织肿块,肿块呈圆形、类圆形或不规则形。CT平扫肿块呈等密度19例,呈高密度2例,增强动脉期轻中度强化,实质期、延迟期明显强化,所有病例增强后均强化不均,16例病灶内见斑片状出血坏死区,3例病灶内有钙化,2例可见病灶周围水肿表现,2例侵犯邻近骨质。MRI上表现为巨大软组织肿块,T1WI呈等低信号,T2WI为中等、高信号或不均匀混杂信号,DWI呈高信号,增强肿瘤实质部分明显强化。结论:CT能够很好地显示SFT病灶及其与周围结构的关系,MRI的信号变化能够反映病灶组织学成分的不同,CT和MRI对于SFT病灶的早期发现、诊断与鉴别、判断病灶与周围组织结构的关系方面具有重要意义。 Objective:To investigate the CT and MRI features of solitary fibrous tumor (SFT). Methods: 21 cases of SFT confirmed by surgery and pathology were analyzed retrospectively. All 21 patients underwent CT,and 8 patients under- went MRI scanning. The location, size, shape, density, signal intensity and enhancement pattern of tumor were studied. Results:The largest dimension of SFT ranged from 3cm to 22. 6era (mean 9. 8cm), showing oval, round or irregular in shape,with ill-or well-defined margin soft tissue mass,iso-attenuated (19 cases) and hyper-attenuated (2 cases) on unen- hanced CT. After contrast administration,tumor showed mild-moderate enhancement in the arterial phase,marked enhance ment in the parenchymal phase and delayed phase. Heterogeneous enhancement could be seen in all patients,with patchy, hemorrhagic and necrotic loci in 16 cases,calcification in 3 cases,peri-lesional edema in 2 cases and bone destruction in 2 ca- ses. Huge soft tissue mass could be revealed on MRI, showing hypo- or iso-intensity on T1WI and intermediate to hyper- in- tensity or heterogeneous intensity on T2 WI, hyper-intensity on DWI and marked enhancement of solid components after con- trast. Conclusion=The mass lesion of SFT and its relationship with surrounding tissue could be clearly displayed on CT. Dif- ferent histological components of the tumor could be revealed by the signal intensities on MRI. CT and MRI are valuable in early detection,diagnosis and differential diagnosis of SFTs,as well as estimating the relationship between tumor and adja- cent structures.
