缺血性脑卒中患者Essen评分与Hcy水平的相关性研究 被引量:2

A correlation study of ischemic stroke patients' Essen score and Hcy level

摘要 目的研究分析缺血性脑卒中患者Essen评分与同型半胱氨酸(Hcy)水平的相关性,以期制定出具有针对性的个体化的治疗方案,早期采取有效的预防措施。方法选取119例缺血性脑卒中患者,根据患者初次入院时的临床资料为患者进行Essen卒中风险评分量表(ESRS)评分,并记录患者缺血性脑卒中的部位。根据量表评分,将入组患者分为低危组,高危组;根据Hcy水平分为高Hcy组与正常Hcy组;并根据缺血性脑卒中的部位分为前循环组,后循环组。分析Essen卒中复发量表评分各组患者的Hcy水平以及前后循环脑梗死患者Essen评分和血浆hcy水平的比较。结果 (1)Hcy≥15 umol/L的缺血性卒中患者中Essen卒中评分<3分的为20.40%,而≥3分的为58.57%,两者比较差异有非常显著性(P<0.01)。(2)根据脑梗死的部位前循环为94例,后循环为25例。前后循环Essen评分分别为(2.89±1.91)、(3.68±1.81),两者统计学上无差异(P>0.05)。前后循环Hcy浓度分别为(16.94±12.76)、(17.60±+7.29),两者统计学上也无差异(P>0.05)。结论血浆同型Hcy水平随着Essen评分增高而升高,其有可能预测缺血性脑卒中发作。 Objective To analyze the correlation of ischemic stroke patients' Essen score and homocysteine(Hcy) level and set up effective measures for treatment and prevention of stroke.Methods 119 patients with ischemic cerebral stroke were selected.Essen stroke risk scores(ESRS) were taken based on the clinical data of their first admission and their positions of the stroke were recorded.The patients were then divided into a group of low risk and a group of high risk.Based on Hcy level,the patients were divided into a high Hcy group and a normal Hcy group.Also,ischaemic stroke positions were taken into consideration to divide the patients into an anterior circulation group and a posterior circulation group.Hcy levels and Essen scores of each group were analyzed and compared.Results(1) For ischemic stroke patients with Hcy≥15umol/l,those with Essen stroke scores 3 accounted for 20.40%,while those ≥3 were 58.57%.The difference was significant(P〈0.01).(2) There were 94 cases with anterior circulation position of cerebral infarction and 25 cases with posterior circulation.The Essen scores for the two groups were 2.89 ±1.91 and 3.68 ±1.81 respectively.There was no statistical difference(P〉0.05).The Hcy levels for anterior and posterior circulation types were 16.94 ±12.76 and 17.60 ± 7.29 respectively,also with no statistical difference(P〉0.05).Conclusion Plasma Hcy levels increase with Essen scores,which may predict ischemic stroke.Plasma Hcy level and Essen scores have no obvious correlation with the stroke position.
