摘要
目的:观察复方丹参滴丸联合阿托伐他汀辅助治疗对早期高血压肾病患者尿微量蛋白(mALB)的影响。方法:将116例早期高血压肾病患者以随机抽样法分为治疗组与对照组,治疗组与对照组最终进入评价者分别为51、46例。2组均采用氨氯地平片5～10 mg·d-1常规治疗,治疗组加用阿托伐他汀10 mg·d-1+复方丹参滴丸1次10丸、1日3次。治疗前及治疗6个月结束时分别测定患者mALB、免疫球蛋白G(IgG)、α1微球蛋白(α1-MG)、β2微球蛋白(β2-MG),进行疗效评价。结果:早期高血压肾病患者通过有效控制血压,可使尿中mALB、α1-MG、β2-MG的含量及检测阳性率有所降低,但除β2-MG检测阳性率下降有临床意义外(P<0.05),其他各项指标治疗前后差异均无统计学意义;在控制血压的基础上加用复方丹参滴丸和小剂量阿托伐他汀,可使mALB、α1-MG、β2-MG含量显著下降(P<0.05),尤其是mALB、β2-MG含量的下降明显优于单独控制血压的患者(2组比较,P<0.05);mALB、α1-MG、β2-MG检测阳性率也进一步下降,尤以α1-MG、β2-MG检测阳性率下降最为明显(P<0.01)。治疗组与对照组有效率分别为56.9%、28.3%,差异有统计学意义(P<0.05)。结论:复方丹参滴丸和小剂量阿托伐他汀辅助治疗早期高血压肾病患者,对疾病进展有一定的抑制作用。
OBJECTIVE：To evaluate the effect of compound Danshen dripping pills plus atorvastatin on urine microalbumin（mALB） in patients with early hypertensive renal disease.METHODS： A total of 116 patients with early hypertensive renal disease were randomized to either treatment group or control group,with 51 cases in treatment group and 46 in the control group included in our final evaluation.Both groups were routinely given amlodipine tablet at a dosage of 5-10 mg·d-1,however,the treatment group received add-on atorvastatin 10 mg·d-1 plus compound Danshen dropping pills（10 pills each time tid）.Determination of microalbumin（mALB）,immunoglobulin G（IgG）,alpha 1 microglobulin（α1-MG） and beta 2 microglobulin（β2-MG） in urine were scheduled at 0 and 6 months and after the completion of the treatment followed by evaluation of curative efficacy.RESULTS： For patients with early hypertensive renal disease,the effective blood pressure control was associated with lower positive detection rate of mALB,α1-MG and β2-MG contents in urine.Among the indexes,only the reduction of β2-MG positive detection rate showed statistical differences（P0.05）,whereas the reductions for all the other indexes showed no significant difference.The addition of compound Danshen drop pill plus small dose atorvastatin to blood pressure control therapy was associated with further reduction of mALB,α1-MG and β2-MG in urine（P0.05）,much as in the reduction of mALB and β2-MG as compared with blood pressure control alone group（P0.05）,and associated with further reduction of positive detection rate of mALB,α1-MG and β2-MG contents in urine,much as in the reduction of α1-MG and β2-MG（P0.01）.The effective rate in the treatment group and the control group were 56.9% and 28.3%,respectively,and the differences were significant（P0.05）.CONCLUSION： Compound Danshen dripping pills plus small dose of atorvastatin as an adjuvant therapy inhibited disease progression in patients with early hypertensive renal disease.
