摘要
目的 规范胃恶性肿瘤的外科治疗 ,提高胃恶性肿瘤的治疗效果。方法 结合作者多年的经验 ,介绍国内外有关胃恶性肿瘤外科规范化治疗的进展。结果与结论 在我国 ,改进和规范胃癌的手术方法 ,推广标准术式 ,具有重要现实意义 ;胃癌根治术应达到充分切除原发病灶 ,彻底廓清胃周围淋巴结 ,完全消灭腹腔内游离癌细胞 3点要求 ;凡发现有浆膜浸润者 ,术中可采用腹腔内高温化疗预防腹膜种植 ;若肿瘤未侵及邻近组织器官 ,局部切除是治疗胃平滑肌肉瘤的常用方法 ;
Objective To standerdize the surgical treatment of gastric neoplasm to get better treatment effect. Methods Introducing home abroad advance in the standerdization of surgical management of gastric neoplasm in the world and author's experience. Results and Conclusions (1)It is important to improve and extend the standerdizing surgical procedure for gastric carcinoma(GC) in China. (2)In surgical treatment of GC, the following purpose should be reached: removing the cancer entirely, dissecting of the lymph nodes around stomach completely and eliminating the intraperitoneal free cancer cells absolutely. (3)Intraperitoneal hyperthermochemotherapy should be done with the serosa of stomach involved by GC. (4)Local resection is a way to treat gastric leiomyosarcoma without involvement of the adjacent organs of stomach. (5)Gastrectomy plus systemic chemotherapy is the best choice for malignant gastric lymphoma.
