摘要
目的探讨腹腔镜下子宫肌瘤剥除术的临床效果和对患者生活质量的影响。方法选择100例子宫肌瘤患者,随机分为观察组和对照组各50例。对照组采用传统开腹子宫肌瘤剥除术,观察组采用腹腔镜下子宫肌瘤剥除术,观察2组手术时间、术中出血量、术后排气时间以及生活质量[世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评分]变化。结果观察组术中出血量、术后排气时间分别为(50.92±11.82)ml和(23.84±3.84)h,明显低于对照组的(75.56±22.09)ml和(34.12±7.55)h,差异有统计学意义(P<0.01);观察组的手术时间稍长于对照组,但差异无统计学意义(P>0.05)。2组术前WHOQOL-BREF各领域评分差异无统计学意义(P>0.05),术后6个月观察组生理领域、心理领域、社会领域、环境领域和主观感受评分分别为14.12±1.51、14.32±1.28、14.76±1.51、14.52±1.29、14.04±1.67,显著高于对照组的13.28±1.21、13.52±1.26、13.88±1.39、13.68±1.22、12.56±1.47(均P<0.01)。结论腹腔镜下子宫肌瘤剥除术的临床疗效优于传统开腹手术方式,且明显提高患者的生活质量。
[Objective]To investigate the clinical efficacy of laparoscopic myomectomy and its influence on quality of life in patients with uterine myoma.[Methods]100 patients with uterine myoma were randomly divided into the observation group and the control group,50 cases in each group.The patients in the control group were treated with myomectomy by laparotomy,and the observation group received laparoscopic myomectomy.The operating time,intraoperative blood loss,anal exhaust time and quality of life（WHOQOL-BREF score） were observed in two groups.[Results]The intraoperative blood loss and anal exhaust time in the observation group was（50.92±11.82） ml and（23.84±3.84） h respectively,which were lower than that in the control group[（75.56±22.09） ml and（34.12±7.55） h]significantly（P0.05）.There was no significant difference in WHOQOL-BREF score before operation between two groups（P0.05）.The score of physical domain,psychological domain,social relation domains,environmental domain and subjective feeling in 6 months after operation in the observation group was respectively 14.12±1.51,14.32±1.28,14.76±1.51,14.52±1.29 and 14.04±1.67,which was higher than that in the control group（13.28±1.21,13.52±1.26,13.88±1.39,13.68±1.22 and 12.56±1.47） significantly（P0.05 all）.[Conclusion]Laparoscopic myomectomy is uperior to myomectomy by laparotomy in the clinical efficacy and quality of life in patients with uterine myoma.
