无创通气改善慢性阻塞性肺疾病急性加重期凝血功能研究 被引量:3

摘要 目的探讨慢性阻塞性肺疾病(COPD)急性加重期患者的凝血功能变化及无创通气对其的疗效。方法观察组30例COPD急性加重期患者给予无创通气,对照组为本院健康志愿者20例。比较观察组及对照组凝血功能,观察组无创通气治疗前后凝血和血气分析的变化。结果无创通气前观察组与对照组比较血浆D-二聚体(DD)、纤维蛋白原(FIB)、凝血酶-抗凝血酶复合物(TAT)、二氧化碳分压明显升高,氧分压明显降低,差异有统计学意义(P<0.01);无创通气后观察组患者二氧化碳分压,DD、FIB、TAT明显降低,氧分压明显升高,治疗前后比较差异有统计学意义(P<0.05)。结论无创通气能改善COPD急性加重期患者存在高凝状态,防止肺小动脉血栓形成及肺动脉高压。 Objective To investigate chronic obstructive pulmonary disease (COPD) in patients with acute exacerbation of the changes of coagulation function and noninvasive ventilation on its efficacy. Metheds The observation group 30 cases of acute exacerbation of COPD patients with noninvasive ventilation, the control group for the 20 cases of healthy volunteers. Comparison of the observed group and the control group of blood coagulation function and observation group of noninvasive ventilation in the treatment of coagulation and blood gas analysis before and after the change. Results The observation group before treatment compared with the control group, the plasma D two dimers (DD), fibrinogen (FIB), thrombin antithrombin complexes (TAT). Partial pressure of carbon dioxide was significantly elevated, oxygen partial pressure was significantly reduced, the difference was statistically significant (P 0.05); noninvasive ventilation after partial pressure of carbon dioxide in the observation group, DD, FIB, TAT decreased significantly, PO2 increased significantly, before and after treatment had a significant difference (P 0.05). Conclusion Noninvasive ventilation for acute exacerbations of COPD can improve the presence of high coagulation state, preventing pulmonary artery thrombosis and pulmonary artery hypertension.
