摘要
目的比较平心定志汤合并维思通与单纯应用维思通治疗以阴性症状为主的精神分裂症的临床疗效与安全性。方法将77例符合CCMD-3精神分裂症诊断标准,并以阴性症状为主的精神分裂症患者,随机分为研究组(38例)与对照组(39例)。对照组应用抗精神病药物维思通常规治疗,研究组在对照组基础上合并应用自拟固定方剂平心定志汤治疗,进行为期12周的临床研究。用阳性与阴性症状量表(PANSS)、阴性症状评定量表(SANS)与治疗时出现的症状量表(TESS)作为评价工具,分别于治疗2,4,8,12周末评定临床疗效及不良反应。结果 2组治疗12周末总体疗效相当,但研究组在治疗4,8周末时显效率及有效率均显著高于对照组(P均<0.05)。研究组治疗4,8周末PANSS总分、阴性因子分、一般病理分及SANS总分均显著低于对照组(P均<0.01)。治疗12周末仅PANSS总分及阴性因子分显著低于对照组(P均<0.01)。研究组的不良反应发生率低于对照组(P<0.05)。结论平心定志汤合并维思通治疗以阴性症状为主的精神分裂症临床疗效与单纯应用维思通治疗相当,但前者起效更快,不良反应更少。
Objective It is to observe the clinical effect and safety of Pingxin Dingzhi Decoction（PXDZD） combined with risperdal in treating negative symptoms of schizophrenia.Methods A randomized clinical trial was conducted.All 77 patients with negative symptoms dominated schizophrenia who met the CCMD-3 diagnostic criteria for schizophrenia were randomized to receive 12 weeks treatment with either PXDZD（test group,n=38） or placebo（contol group,n=39） added on risperdal.The efficacy was assessed with measurement including the Positive and Negative Symptoms Scale（PANSS） and Scale for the Assessment of Negative Symptoms（SANS）.The Safety and tolerability were assessed with the Treatment Emergent Symptom Scale（TESS）.Scales were evaluated at the ends of the 2nd,4th,8th and 12th week.Results At the end of the 12th week,the curative effects of both groups were similar.But the excellence rates and effective rates in test group were significantly higher than that in control group at the end of the 4th and 8th week（all P0.05）.The total score of PANSS and SANS,single item score of SANS and routine pathology at the end of 4th and 8th week were markedly lower than that in control group（all P0.01）.The total score of PANSS and single item score of SANS in test group at the end of 12th week were lower than that in control group（P0.01）.The incidences of adverse reactions in test group were significantly lower than that in control group（P0.05）.Conclusion There curative effect of PXDZD plus risperdal are similar to single risperdal in negative symptoms dominated schizophrenia,but with earlier become effective and less side effects.
