1,5-脱水葡萄糖醇在初诊2型糖尿病患者血糖监测中的价值 被引量:3

The role of monitoring plasma 1,5-anhydroglucitol in newly diagnosed type 2 diabetic patients

摘要 目的评估1,5-脱水葡萄糖醇(1,5-AG)在初诊2型糖尿病(T2DM)患者血糖监测中的价值。方法选择初发T2DM患者192例,通过糖尿病教育、营养指导、调整口服药物或胰岛素剂量使血糖达标;比较治疗前和开始治疗后第2、4、8周患者的空腹静脉血糖(FPG),1,5-AG,果糖胺以及糖化血红蛋白(HbA1c)的变化。结果治疗2、4周时的1,5-AG为(5.40±2.81)μmol/L、(9.05±3.07)μmol/L,均明显高于治疗前的(1.86±1.30)μmol/L(P<0.05);HbA1c于治疗4周时开始下降。治疗8周时,血清1,5-AG与HbA1c、果糖胺、FPG水平呈负相关(r分别为-0.576、-0.594、-0.663,P<0.01)。结论 1,5-AG比HbA1c反应灵敏,能快速反应血糖变化,可用于T2DM患者早期的血糖监测。 Objective To evaluate the significance of monitoring serum 1,5- anhydroglucitol (1,5-AG) in newly diagnosed type 2 diabetes(T2DM). Methods A total of 192 patients with newly diagnosed T2DM was treated with diabetes education, nutritional counseling and oral antihyperglycemic medications or insulin to keep blood glucose at a target level. Serum levels of 1,5- AG, HbAlc, fructosamine, and fasting plasma glucose(FPG) were monitored before and at 2,4 and 8 weeks during treatment. Results Serum levels of 1,5-AG at 2 and 4 weeks were (5.40 ± 2.81)μmol/L and (9.05 ± 3.07) μmol/L, respectively, which were higher than (1.86 ± 1.30) μmol/L before(P〈0. 05). Serum HbAlc began to decrease at 4 weeks of therapy. At 8 weeks of therapy, the elevation of serum 1,5-AG was negatively correlated to the decrease of HbAlc, fructosamine and FPG (r=-0. 576,-0. 594 and -0. 663,P〈0. 01). Conclusion Compared to HbAlc examination,detection of serum 1,5-AG is more sensitive in rapidly reflecting the changes of blood glucise and may be taken as an indicator for monitoring of T2DM patients in early stage.
