自体骨软骨移植治疗膝关节全层软骨损伤的疗效评价 被引量:5

Repair of Full-Thickness Articular Cartilage Defects of Knee Weight-Bearing Surface Using Osteochondral Autologous Transplantation

摘要 目的评价镶嵌式自体骨软骨移植治疗膝关节负重区全层软骨损伤的手术方法和临床疗效。方法对21例膝关节负重区全层软骨损伤患者采取镶嵌式自体骨软骨移植治疗,术后随访采用膝关节Lysholm评分和Tegner运动评级进行评价。结果所有患者均获得随访,在术后9个月时,Lysholm评分由术前20~55(38.50±7.25)提高到术后53~100(65.30±6.20)。Tegner评级由术前1~3(1.55±0.62)提高到术后3~6(3.58±0.80)。术前、术后差异有统计学意义(P<0.01)。结论自体骨软骨移植是治疗膝关节全层软骨损伤的一种安全有效的方法,而且中远期疗效满意。 Objective To evaluate the outcomes of osteochondral autologous transplantation(OAT) for the full-thickness articu- lar cartilage defects of the knee joint weight-bearing surface. Methods Twenty-one patients with full-thickness articular cartilage defects of the knee weight-bearing surface were treated by mosaic-type OAT. The clinical assessment was performed using Lysholm scores and Tegner scores. Results All patients were followed up. At the end of ninth month after operation, the mean Lysholm scores were as follows : preoperative ,38.50 ± 7.25 ( range ,20 - 55 ) ; postoperative, 65.30 ±± 6.20 ( range, 53 - 100 ) ; P 〈 0.01. the mean Tegner scores were as follows : preoperative, 1.55 ± 0.62 ( range, 1 - 3 ) ; postoperative,3.58 ± 0.80 ( range,3 - 6) ,P 〈 0.01. Preoperative and postoperative difference was statistically significant. Conclusion Osteoehondral autologous transplanta- tion is a safe and clinically effective procedure, which has been proven to be valuable in treating osteochondral defects even over the long term.
