摘要
目的探讨支气管肺泡灌洗对慢性阻塞性肺疾病急性加重并呼吸衰竭治疗的影响。方法以我院与2007年1月—2011年12月收治的COPD合并急性加重并呼吸衰竭患者为研究对象,随机分为两组。对照组患者进行有创-无创机械通气;治疗组在有创-无创机械通气的基础上,加用支气管肺泡灌洗并注药,观察其疗效。结果配合纤维支气管镜灌洗及注药的相关疗法可有效引流呼吸道炎性物质,使得通气功能得以改善,使有创-无创机械通气切换时间窗得到缩短,对选择抗生素更有针对性。结论对AECOPD并呼吸衰竭的患者,采用支气管肺泡灌洗能有效引流;针对PSB的采样培养结果采取有效的抗感染治疗;能缩短拔管周期,能尽量在72h以内拔管并改行无创机械通气,防止VAP的发生。
Objective To investigate the bronchial alveolar lavage of chronic obstructive pulmonary disease with acute exacerbation and respiratory failure, the impact of treatment. Methods An object of study is that our hospital merge with acute exacerbation of COPD which admitted from the January 2007 to December 2011, and patients with respiratory failure as the research object were divided into two groups randomly. The control group patients will make invasive and non-invasive mechanical ventilation; treat- ment group is based on the invasive and non-invasive mechanical ventilation, add to bronchial lavage and injection to evaluate efficacy. Results It is can draining the respiratory tract inflammatory substances effectively that we coordination fiberoptie bron- choscopy lavage and injection related therapy , making the ventilation function will be improved, so that invasive to non-invasive mechanical ventilation switching time will to be shortened and more focused on the choice of antibiotics. Conclusion For AECOPD and respiratory failure patients; It can be effective drainage that we gdopt hrunchoalveolar lavage, take effective anti-infection treatment for PSB sampling culture results; shorten the extubation period, as far as possible within 72 hours extubation and divert- ed to non-invasive mechanical ventilation to prevent the incidence of VAP.
