摘要 目的探讨吸烟对心率变异性和QT间期、QT离散度的影响。方法选择2010年1月~2011年12月,本院门诊的健康体检者,根据是否吸烟分为不吸烟和吸烟二组,每组100例,共200例。均接受24小时动态心电图检查,以人机对话方式识别各种心律失常,剔除异位搏动和伪差,自动检出窦性心搏并计算SDNN、SDANN、SDNNindcx、rMSSD、PNN50,同时利用计算机辅助测量QT间期,取平均值并计算QT离散度。结果与不吸烟组比较,吸烟组心率变异性指标的SDNN、rMSSD及PNN50显著降低,P<0.05~0.01;吸烟组的QT间期显著延长,QT离散度显著增大,P<0.05。结论吸烟可显著降低心率变异性;延长QT间期,使QT离散度显著增大,这些可能是吸烟增加心血管事件的机制之一。 Objective To investigate the effect of smoking on heart rate variability (HRV), QT interval, and QT dispersion. Methods Two hundred healthy persons who received physical examination in our hospital from January 2010 to December 2011 were divided into smoking group (n = 100) and non-smoking group (n = 100). These persons were monitored with 24-h dynamic electrocardiogram. Various types of arrhythmia were detected by man-machine interaction, and ectopic beats and artifacts were removed. Sinus rhythm was automatically detected, and SDNN, SDANN, SDNN index, rMSSD, and pNNS0 were calculated. QT intervals were acquired by computeraided measurement, and their mean value was obtained. QT dispersion was calculated. Results Compared with the non-smoking group, the smoking group showed significantly decreased SDNN, rMSSD, and pNNS0 (P〈0.01-0.05) and significantly increased QT interval and QT dispersion (P〈0.05). Conclusion Smoking can significantly decrease HRV, prolong QT interval, and increase QT dispersion. That may be one of the mechanisms by which smoking increases the incidence of cardiovascular events.
