摘要
目的探讨子宫癌肉瘤的CT及MRI表现特点,提高对该病的诊断水平。方法回顾性分析5例经手术病理证实的子宫癌肉瘤的临床资料及CT、MRI表现。其中3例行CT平扫及增强,2例行MRI平扫及增强。结果 2例CT平扫表现为宫腔内卵圆形等低密度肿块影,密度均匀,未见囊变及钙化,边缘光滑。1例表现为子宫弥漫性增大,呈不规则等密度肿块,内见囊变坏死。2例MRI平扫表现为子宫腔明显扩大,内见巨大肿块,T1WI呈等信号,T2WI呈不均匀高信号,1例病灶内可见长T2信号的小囊样坏死区。2例子宫结合带显示清晰。增强后5例呈不均匀强化,4例可见轻度及明显强化区域,静脉期病灶内动脉期明显强化区域呈渐进性延迟强化,但稍低于周围子宫肌层。1例右侧附件区转移,2例腹膜后淋巴结转移,1例肝转移,1例伴有胸腹盆腔积液。结论子宫癌肉瘤CT及MRI表现有一定特征,特别是增强后的强化特征能够反映其病理特点,有助于对其诊断及鉴别诊断。
Objective To investigate CT and MR features of carcinosarcoma of the uterus and promote its diagnostic accuracy. Methods Clinical data, CT and MR manifestions of five cases with uterine carcinosarcoma confirmed by operation and pathology were reviewed retrospectively. Of five cases, plain and enhanced CT was performed in 3 cases, plain and enhanced MR was performed in the other 2 cases. Results A well defined oval homogeneous low or moderate density mass within the uteric cavity was seen in 2 cases on plain CT, no cyst or calcification was noted, and an irregular moderate density mass with intratumoral cystic change and necrosis within diffuse enlargement of the uterus were revealed in the one case. A huge T1W isointensity and T2W mixed hyperintensity mass within uterus which was markedly enlarged was seen on two cases, one of them showed small cystic long T2 necrotic area within tumor on T2WI. Two cases demonstrated clear conjunctive area of uterus. Heterogeneous enhancement masses were seen in all the five cases. Mild and marked mixed enhanced areas and gradual enhancement with a tendency from the arterial phase to the venous phase were revealed within tumors in four cases. Metastasis to right ovarian area was noted in one case, retroperitoneal lymphadenopathy in 2 cases, hepatic metastasis in 1 case, and associated with pleural effusion, ascites, and pelvic effusion in 1 case. Conclusion There are CT and MR features of uterine carcinosarcoma, especially the enhancement pattern is correlated with pathologic characteristics, and it will be very helpful for diagnosis and differential diagnosis.
